By checking the box below, you the parent/guardian of the student(s) (listed above), hereby grant permission to Cold Lake Community Church (CLCC) Youth to post on Social Media any photos or videos that may be taken of your child(ren) at Amy CLCC Youth sponsored event. These photos may be used for promotion, or any other information that may be presented by CLCC Youth, including, but not limited to, website promotion, and Instagram and Facebook Posts.
