  • Embry Chapel is celebrating its 115th Church Anniversary. Thank you for your support to help us celebrate this event and to create a lasting memory for our entire church family.  We are asking your support through the purchase of an Ad for our Souvenir Booklet. Please complete this form and submit your ad(s) to embry115thanniv@embrychapel.org no later than April 28, 2023; or you may send your AD, payment, and completed form to Embry Chapel AMEC, 264 30th Street, Ogden, UT 84401.

    The souvenir booklet will be available during the Sunday Celebration.  Anyone making a donation of at least a quarter-page ad will receive a complimentary booklet.

  • Ad(s): You may attach a pre-printed and/or pre-type set ad with/without photos but for optimal quality of your Ad to be included in the Souvenir booklet, please send pictures in the format of JPG, TIFF, or BMP, and if you have text to be included with your picture.
