Allegany Event Rentals Liability Waiver

We strive to provide a safe, clean, and fun atmosphere for you and your children, PLEASE READ THE FOLLOWING carefully before signing this waiver of liability. On behalf of myself, my children and children in my care participating in activities provided by Allegany Event Rentals,

I acknowledge and agree to the terms and conditions of this waiver as identified below:

1. I and the children in my care who will participate in any Allegany Event Rentals activities are physically, mentally, and emotionally fit to participate in such activities.

2. Waiver and Release: Participant understands and acknowledges that this Release discharges Allegany Event Rentals from any liability or claim that the Participant may have against Allegany Event Rentals with respect to any bodily injury, personal injury. illness, death, or property damage that may result from Participant's use and possession of Allegany Event Rentals equipment.

3. Participant does hereby release and forever discharge and hold harmless Allegany Event Rentals and its successors and assigns from, any and all, liability claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Participant's possession and use of Allegany Event Rentals equipment.

4. Assumption of the Risk: There are risks, both known and unknown, using any inflatable or amusement device, including, but not limited to physical injury, emotional injury. distress, paralysis and even death. The risk of serious injury from participating in Allegany Event Rentals's activities, although minimal, does exist, as it does in all play and amusement activities. Participant understands and acknowledges that this Release discharges Allegany Event Rentals from injury. illness, death, or property damage resulting from the possession and use of Allegany Event Rentals equipment. Participant recognizes and understands that use of Allegany Event Rentals equipment may involve inherently dangerous activities.

5. Insurance: Participant understands that insurance for liability, health, and medical or disability coverage in any way related to the rental of equipment under this agreement is the sole responsibility of Participant. 6. Indemnification: Participant agrees that he/she will indemnify and hold Allegany Event Rentals harmless from any liability resulting from the use of Allegany Event Rentals equipment by Participant, his/ hers assigns, heirs, guests, invites, or other persons using the equipment while in Participants possession.

7. Other: Participant expressly agrees that this Release is intended to be as broad and inclusive as permitted. VOLUNTARILY AGREE TO THESE TERMS. The Participant does hereby freely, voluntarily. and without duress executes this Release under the noted terms.
