Informed Consent and Acknowledgement
I hereby give my approval for my details provided to be used in collaboration for my registration to the community and all activities prepared by {Organization} .
In case of injury, I hereby waive all claims against {Organization} . Including all affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. Only the person responsible for said injury/accident, if relevant, may be liable. There is a risk of being injured that is inherent in many activities you may part take in throughout the years of your membership. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death. Please be vigilant and pay close attention to any safety briefing before any event.
