This reference should be completed by a Minister, Pastor, Network Leader or Youth Leader. In each case the referee must have known the applicant personally for a minimum of twelve months. Please note that this document is treated with the highest level of confidentiality which allows you the freedom to comment openly on the applicant. If a Pastor is directly related to the applicant, we require at least one Pastoral Reference Form from a Church Leader not related to the applicant.
