This form is used when you have transferred a property that you owned personally to a corporation or an LLC. Do NOT use this form if you have actually sold the property to a new owner and you no longer have an ownership interest in the property.
Prior to submitting this form, you will need to have completed the legal steps necessary to transfer ownership of the property. This usually entails a quitclaim deed or some other form of deed transfer that will need to be recorded with the county records office. Contact a real estate attorney if you need assistance with that, or we can refer you to one we recommend. Only submit this form AFTER the paperwork has been completed, as we need a copy of the paperwork before we can do anything on our end.
