    We would love it if you shared your story through a video. By sharing your story through a video it allows you to share your voice. Below are some guidelines for your video.
  • LENGTH - Videos should be at most 2 minutes long.

    LIGHTING - When creating a video you want to make sure you are filming in a room that has the most natural light. Do not film in an area with harsh overhead lighting or strong artificial light sources that can create unflattering shadows or color casts.

    SOUND - Choose a room with minimal echo and background noise to ensure clear, high-quality audio recordings. 

    CAMERA POSITION - When you are filming, position your camera in a way that we can see from the torso up or from the shoulders up. Please record video with your phone vertical.

  • I hereby grant and authorize Lung Cancer Initiative the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me or the minor(s) associated with this agreement to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits, and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats, and markets now known or hereafter devised. This authorization shall continue indefinitely unless I otherwise revoke said authorization in writing.

    I understand and agree that these materials shall become the property of Lung Cancer Initiative and will not be returned.

    I hereby hold harmless, and release Lung Cancer Initiative from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.
