Questions to Answer In Your Story

• Where are you from?
• When were you diagnosed with lung cancer?
• Which type of lung cancer were you diagnosed with?
• What kind of emotions did you experience when you received your diagnosis?
• Are you currently receiving treatment? If not, when did you go into remission?
• Who was or is the biggest supporter for you during this time?
• What has inspired you along your journey?
• Where have you found your strength?
• How has Lung Cancer Initiative helped you with your journey with lung cancer?
• Has your perspective on life and health changed since your diagnosis?
• What is the best piece of advice you would give to someone who has been recently diagnosed with lung cancer?

LENGTH - Videos should be at most 2 minutes long.

LIGHTING - When creating a video you want to make sure you are filming in a room that has the most natural light. Do not film in an area with harsh overhead lighting or strong artificial light sources that can create unflattering shadows or color casts.

SOUND - Choose a room with minimal echo and background noise to ensure clear, high-quality audio recordings. 

CAMERA POSITION - When you are filming, position your camera in a way that we can see from the torso up or from the shoulders up. Please record video with your phone vertical.

I hereby grant and authorize Lung Cancer Initiative the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me or the minor(s) associated with this agreement to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits, and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats, and markets now known or hereafter devised. This authorization shall continue indefinitely unless I otherwise revoke said authorization in writing.

I understand and agree that these materials shall become the property of Lung Cancer Initiative and will not be returned.

I hereby hold harmless, and release Lung Cancer Initiative from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.
