    Join us for a fast-paced, competitive, and rewarding basketball camp experience for players of all levels. Quality instructors providing a safe environment for players to expand their games mentally and physically.

    We will have two high school size courts with 1 court for novice/intermediate and court 2 intermediate/advance players!

  • NORWOOD Policy Agreement

    The information I have provided is accurate to the best of my knowledge. I am authorizing a non-refundable deposit of $150 per child, per session to reserve my child's space in camp. I agree to pay the balance of the tuition in full by May 12, 2023 or my space(s) will be forfeited. I also understand that no refunds will be granted after May 12, 2023.

     

    The Camp reserves the right to suspend or dismiss any camper at any time if the actions of a camper or parent/guardian make a positive and constructive relationship impossible. 

     

    I give permission for photos of and artwork by my child to be used in Summer at Norwood publications, social media, or advertisements.

     

    COVID-19 is extremely contagious and is believed to spread by person-to-person contact; and, as a result, federal and state health agencies recommend social distancing and various other measures to mitigate the risk of contracting the virus.

    Despite the health and safety protocols outlined above and other measures in place to mitigate the risk of transmission of COVID-19, there are inherent risks that students and/or their families may become infected with COVID-19 due to the student's participation in Summer at Norwood.  These risks include, but are not limited to, the following: exposure to COVID-19, becoming infected with COVID-19, or becoming a symptomatic or asymptomatic carrier of the virus.  It is possible that a student is already an asymptomatic carrier of the virus before and that the student may infect other students, employees, or other individuals at the School as a result of their attendance. Any person who receives a positive diagnosis of COVID-19, including a camper or their family member, may encounter extended quarantine/self-isolation, additional tests, medical care, hospitalization, other potential complications, and the risk of death. 

     

    By signing the Registration Agreement, parent(s) agree to adhere to the policies and procedures outlined by Summer at Norwood.  By allowing their child(ren) to attend Summer at Norwood, parents understand, acknowledge and voluntarily assume these risks on behalf of themselves and the camper(s).

     

     

    Statement of Wellness for Participation and Permission to Administer Emergency Treatment

    I do hereby verify that my child to the best of my knowledge is free from contagious disease, is fully immunized or has immunization exemptions because of parental or guardian objection or medical contraindication, and is able to participate fully in the summer programs. In the case of a medical emergency and the event that the parent/guardian cannot be reached, I hereby give my permission for emergency treatment to be administered to my child. I understand that Summer at Norwood does not carry accidental injury insurance on campers and I waive and release Summer at Norwood and its employees from all liability for any personal injuries, illness, loss, or damage to property. I agree to assume liability for any expenses incurred in such an emergency (transportation, hospitalization, x-rays, etc.).

     

  • Waiver/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19
    ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT


    In consideration of being allowed to participate on behalf of (M.A.D.E. Team Sports, INC.) and We (Organization) athletic program and related events and activities, the undersigned acknowledges, appreciates, andWAIVER agrees that:

    1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. Following safety protocols and personal discipline may reduce this risk, the risk of serious illness and death does exist.

    2. We KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others and assume full responsibility for my participation.


    3. We willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, We observe and any unusual or significant hazard during my presence or participation, We will remove myself from participation and bring such to the attention of the nearest official immediately.

    4. We and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (M.A.D.E. Team Sports, INC.) their officers, high school trainers and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

    We HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. WE HAVE MADE OUR PARENTS AWARE OF THIS LIABILITY AND RELEASE AND WILL BE HELD RESPONSIBLE IF WE HAVENT NOTIFIED OUR PARENTS OF THIS LIABILITY WAIVER.

    FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
    This is to certify that We, as parent/guardian/organization, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. We, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
