Hello there, first and foremost thank you for your interest in getting tattooed by me. Please ensure that you are reading through this form carefully and fill out all required info. ** PLEASE NOTE THIS IS A REQUEST FOR AN APPOINTMENT ONLY THIS DOES NOT GAURANTEE AN APPOINTMENT.** I generally book for 2-3 months at a time and as I would love to tattoo all of you sometimes that is just not possible during a specific booking period. Should you be selected, my email response will include instructions as to whether or not we need an in person consult, a rough est. of price and further instructions as to how to leave a deposit and schedule a date. **PLEASE NOTE APPOINTMENTS ARE NOT GUARANTEED UNTIL A DEPOSIT IS LEFT**

My rates: 

HOURLY $200/Hr - 1/2 day sitting(4rs) $750 - 3/4 day(6hrs) $1050

Deposits are $200 and will come off the total price of the tattoo.(if the tattoo is multiple sittings it will be taken off the last sitting)

**DEPOSITS ARE NON-REFUNDABLE. IF YOU NEED TO RESCHEDULE YOU HAVE UNTIL 72 HOURS PRIOR TO THE DATE TO DO SO AND YOUR DEPOSIT WILL BE TRANSFERED TO YOUR NEW APPOINTMENT DATE. 

 

PLEASE INCLUDE A PHOTO OF THE AREA YOU WOULD LIKE TO GET TATTOOED.

IF THERE ARE ANY IMPORTANT DETAILS OR ELEMENTS   YOU WOULD LIKE ME TO FOCUS ON THAT WILL ASSIST IN. THE DESIGN PROCCESS, PLEASE LET ME KNOW BELOW.
