BACKGROUND: Middaugh Chapter Achievement Awards recognize outstanding programs and projects conducted by the chapters. Nominations are accepted from individuals and chapters. Submit one form for each activity that you wish to nominate. Activities may only be nominated in one category (i.e. a pumpkin sale that promoted agriculture and raised funds for the Chapter/Colony may be entered in either the "Promotion of Agriculture" category or the "Fundraising" category -- not both). DEADLINE: Nominations are accepted until 11:59 p.m. Pacific time on Friday, March 15, 2024.
