In worship, I am encouraged and comforted through the music, prayers, sermon, and friendship.
In worship, I hear God speak to me. What I experience on Sunday carries over into my ability to follow Jesus during the other days of the week. During the service, I experience encouragement as well as challenging invitations.
I believe worship is more about making an offering to God than it is about having my needs or preferences met. Communal worship is essential to my life as a disciple; I also worship God daily as a personal practice. I turn to God in the good times and bad.

Which selection best describes your current experience of the church?

I'm not involved with a church. It's either been a source of hurt for me/my loved ones or it hasn't really seemed relevant to me. In general, I'm searching for a greater sense of belonging and I wonder if the church is a good place to look.
Overall, I'm having positive experiences with a church community. I know some names and basic info of the people at church, and I'd like to make it a more regular part of my life.
My friendships at church are becoming deeper. We talk about both the highs and lows of life. As I'm becoming more involved, I'm learning the importance of relationship.
I believe that participating in the body of Christ - the church - is essential to the life of a Christian. I am called to be a part of this covenant community. Despite differences, Christ's presence and the mission unite us. I'm actively pursuing becoming a church that better reflects the diversity of God's kingdom.

I don't talk to many people I know about faith or spirituality. I wonder how I might develop relationships with other believers.
I would like to have some friends or mentors that can help me grow in my faith and share in honest conversation about our spiritual lives.
My friends know me because of our open and honest sharing. They help encourage my spiritual life and we enjoy fellowship and study together.
My friendships are characterized by our mutual pursuit of Jesus. In order to faithfully follow Jesus, I need friends to hold me accountable, support and challenge me, hear my confessions, and forgive me. I couldn't grow as a disciple of Jesus by myself.

Putting my identity in people, money, appearance, or achievement isn't seeming to cut it anymore. I'm trying to discover who I really am.
I've started to hear and believe that I am a beloved child of God.
I am growing into my identity as a baptized and beloved child of God. I need God's continual reminders of who I really am.
I am firmly grounded in my identity as a child of God. My understanding of myself comes solely from what God says about me and about who I am in Jesus.

I have a general idea of who I am and what I'm good at - it's informed by the things I enjoy, where I've been successful in the past, my skills, and work experience.
I've heard others talk about their spiritual gifts and I'd like to know what mine are. I've had an experience where I learned something about who I really am at the core that surprised me.
I'm practicing growing in my self-awareness of my gifts and weaknesses. I celebrate and live into my gifts. In my weaknesses, I actively turn to God for healing and forgiveness and to others for true reconciliation.
I constantly serve out my gifts and passions. I sense the Holy Spirit empowering my service and bearing fruit when I do this. My life is characterized by practicing reconciliation in relationships.

I serve others through community organizations or my workplace. I think it's a good thing to do and I enjoy giving back.
I volunteer for some organized opportunities; I feel good when I serve and I want to do more of it as a follower of Jesus.
I'm beginning to see service as a main component of being a disciple of Jesus. I serve through organized opportunities and I also try to incorporate serving others into my daily life wherever I am.

I believe that I have experienced God's presence in my life but can't articulate it.
I can name and describe how God has been and is present in my life.
Every day I ask for God's guidance in my life; I expect that God will be present and I rely on God's guidance throughout the day.

I do some things that bring me peace or gratitude or that fill me with wonder for the world.
I'd say I'm a beginner at spiritual practices like prayer, silence, or reading scripture. I'm trying some practices out to see what fits for me.
I am committing to regularly engage in a set of spiritual practices. They are on my daily/weekly schedule and I try to stick to them.
My day is scheduled around a rhythm of spiritual practices - even my work and tasks. I also teach others how to practice spiritual disciplines as a way of experiencing God. I hope my own practices can witness to a new way of life for someone.

Sometimes when a tragedy arises or I am in a tough spot, I saw a prayer, wondering if it will do anything. I offer my thoughts or good vibes to others.
I pray for myself and others, thanking God and asking God to meet our needs.
My prayers are more of a conversation with God, involving both speaking and listening. My prayers for others include those that I am connected with personally as well as those beyond my community or nation.
In prayer, I submit my will to God's will for me and the world. My prayers are shaped by a vision of God's kingdom and I even pray for my enemies. Some of my prayer involves words; a lot of my prayer involves silence.

I don't know much of what's in the Bible other than what I've picked up culturally or learned in my childhood. I'd like to find out for myself what it says, who Jesus is, and why people follow him.
Since I've decided to follow Jesus, I want to learn more about who he was and what his life and ministry looked like.
I read the Old and New Testaments and I see them as a continuation of the same story. I can name some of the major themes of scripture. I read scripture on my own, hear the stories in worship, and read the Bible with others to gain new insight and perspective.
I find myself thinking about and speaking the stories and language of scripture. Approaching scripture to encounter God rather than material brings a new depth to familiar passages of scripture. Seeking out diversity in biblical interpretations is an important practice to me.

I sometimes give when I have extra money after spending and saving.
I have a small line for giving in my budget, or it happens sporadically.
I have pledged to consistently give to the church. I am discerning tithing (giving 10% of my income to God through the church).
I have pledged to give a tithe to the general church offering, and when particular needs arise I give an additional gift.

I spend most of my money on myself or my family, giving a little extra to those in need where I can.
I share my money when the need arises or someone asks me to give. I value giving to others in need because I believe Jesus would help them.
I actively evaluate the way I am using my money and sometimes have to make hard decisions or give something up so that I can give to God or someone else.
I intend for my giving to be sacrificial; I release control of my money once I give it. My top priority in creating my budget is how I can further God's kingdom of peace and justice.

I mostly spend time on myself, my family, or my career. I volunteer my time when asked and if I am available.
I actively evaluate how I am spending my time; I sometimes have to make hard decisions and say no to things that I would enjoy or feel obligated to do in order to live as a disciples of Jesus, including both work and rest.
The way I use my time reflects my worship of God above all other things I could prioritize, I freely give my time to further God's kingdom.

Which selection best describes your perception of God's work in the world?

I wonder where God is. If God exists, God must be pretty distant or hands-off given the state of the world.
I believe that God is active in the world today because I have experienced it and heard others talk about heir experiences. In general, it's hard for me to know what to look for to see God in the world.
I can readily recognize and name moments of God's activity in the world based on what I read in scripture. Stories and themes from the Bible come to mind as I reflect on what I experience in the world; they help me to understand truth and to point to God's presence.
I see God's work in the world everywhere I look. My views of what God's activity looks like have been challenged and changed from what I used to believe - sometimes God's work is uncomfortable, hard to understand, or even painful.

I don't think I have story like this to tell. I'd be interested to hear others' stories.
I have a story of God transforming me but I don't know how to put words to it.
I have a story of transformation and I practice sharing it in worship and in small groups. I continue to notice smaller moments of transformation that contribute to my larger story of faith.
I actively share stories of my ongoing transformation through Christ both inside and outside of my faith communities. I also help others to recognize and share their own stories.
Which selection best describes how invitation to the Christian faith is playing a role in your life?

A friend has invited me to church on Sunday or a church event.
I've personally sensed God living me and inviting me into a relationship with God.
I've personally sensed God loving me and inviting me into a relation with God. This friendship with God leads me to invite others to church services, events, ministries, or missions. But it's hard and doesn't happen often.
I consistently seek to invite others not only into my church community, but to experience a relationbsup with Jesus alongside me.
