I approve and allow For the Love of Dog Rescue to contact personal and veterinary references, I also allow For the Love of Dog Rescue to have information regarding my pets spay/neuter status and vaccination history. I understand that submitting this application foes not guarantee that I will be approved as a foster home.

I understand that For the Love of Dog Rescue reserves the right to remove the foster dog from my care at any given time should the rescue deem my home unfit or unsafe for the dog. I agree to comply with all the foster guidelines and instructions provided by For the Love of Dog Rescue.

I agree not to board, rehome, or give away the foster dog to anybody without prior approval from For the Love of Dog Rescue. I understand that any change in the foster dog’s living situation must be communicated to and approved by For the Love of Dog Rescue.

I agree to provide a safe and loving environment for the foster dog, including proper food, water, shelter, exercise, training, and medical care as instructed by For the Love of Dog Rescue. I understand that any medical expenses incurred for the foster dog must be preapproved by For the Love of Dog Rescue.
