How many people are you expecting to attend the baby dedication?*
Upon completion of the form, the Children's Pastor will call to validate the information, confirm the date of the dedication, and provide additional next steps. If you have any questions/concerns, please email discover@friendshipwest.org.

I hereby grant Friendship-West Baptist Church permission to use videos and photographs of my image and likeness in any of their Web-based publications, Print Advertisements, Social Media, Organization Bulletin, FWBC NEWS, and any Friendship-West Baptist Church Content, whether digital or print.
