I, the customer, agree with the following statements:

This rental equipment has been received in good condition, and I will return equipment in the same condition. I agree to pay in full the replacement cost, including labor, for all damages to rental equipment in the event of property being damaged while in my care.
I will not loan, sublet or otherwise depose of equipment. I will not use equipment at any other location other than the location indicated on contract. I agree to pay a cleaning fee of $150 if equipment is returned dirty. I give consent to charge credit card on file in the event of such statement above.
I understand There are no warranties of merchantability or fitness either expressed or implied. The person/s or organization renting this Equipment will not be held responsible and liable for any and all damage or injuries occurring for any reason whatsoever.
I agree to be charged for an additional day in the event equipment isn't returned by the end of contract agreement.
I will agree to pay a $50 delivery and pickup fee, within a 50 miles radius of M&M Imports location at 1516 Kalamazoo Dr Suite A3 Griffin GA 30224
