The following information is used by Wild Education Tasmania to better uphold and manage the safety of students during any training. It also assists us in making adjustments to individual client needs where possible.

• we guarantee that your personal & medical information is only used in relation to this trip in and will only ever be shared with the appropriate staff and Emergency Services when applicable.
• if you do not disclose an ailment, injury, illness or condition and it presents itself throughout the training we cannot be held responsible and it may inhibit effective treatment. 

Your Medical Form Declaration 

• The medical information you have provided will help our staff better assist you in the event of an accident or illness.
• By signing this form you are acknowledging the information you have provided is accurate and truthful at the time of completion.
• The information you disclose will be kept confidential, and will only be used to assist staff respond to a medical situation.
• If you are under 18 years of age this form must be completed with your parent/guardian & signed off by them.

1. I am fully aware that my participation in this Wild Education Tasmania training activity will involve discussions, presentations and role play on mental health issues.
2. I acknowledge and agree that I am undertaking this mental health training freely, voluntarily and absolutely at my own risk.
3. I understand that it is at my discretion to make Wild Education Tasmania staff aware of any mental health challenges I personally have.
4. I understand that I may be triggered or affected by the content of this course and have the right to remove myself from any training actvities that I do not feel comfortable with.
5. I acknowledge that while Wild Education Tasmania, its staff and/or its partners will make every reasonable effort to minimise exposure to any mental trauma throughout the course, there may be times where this is out of their control due to the content that needs to be covered. 
6. I am aware that if I choose not to participate in the discussions & activities for any reason whatsoever I must inform the instructor as soon as possible. 
7. I am sufficiently fit to participate in this program. I have completed a medical form that is accurate, complete and true to the best of my knowledge. I agree to notify Wild Education Tasmania staff and/or its partners of any changes to my health and mental wellbeing which may occur before or during the program.
8. Should I become ill or injured, I give my consent to the Wild Education Tasmania staff and/or its partners to provide or arrange for provision of medical treatment or rescue services as they see fit.
9. I release Professional Explorers Pty Ltd as the trustee for the Red Hot Go Unit Trust (trading as ‘Wild Education Tasmania’ and 'Wild Journeys) and its officers, employees, agents, licensees, guides, partners and other representatives from all cost, liability, loss or damage incurred or suffered by me directly or indirectly during the course of the training and resulting from my personal injury, illness or death or damage to or loss of my property unless caused by the wilful negligence or wrongful act of Wild Education Tasmania and its employees.

1. I authorise Wild Education Tasmania and/or its partners to use any photographs, video or film taken of myself during the trip and related activities. I understand there will be no compensation to me.
2. I understand that the images, video or film may appear in print, electronic, or video media, and may be available to a global audience through the internet and/or social media.
3. I acknowledge that ownership of the photographs, video or film will be retained by Wild Education Tasmania and/or its partners.
