  • All the following documents must be uploaded before start of your training. We strongly recommend uploading all the documents at the same time to avoid processing delays.

    1. Live Scan Request form signed by a live scan facility. Read Live Scan page for more details. 
    2. Physical Exam Report. Visit Physical Exam page for more details. 
    3. Copy of your TB negative report. Visit TB Screening page for more details.
    4. Copy of your ID. Please upload ANY ONE of the below. Only the front side is needed. 
      • Driver's license, (from CA or any other US state)
      • State ID, (from CA)
      • Passport, (Only US passports are accepted)

    Please make sure your file name is descriptive and includes your name. Example - "John-Taylor-COVID.jpg"
