Thanks for your willingness to make deliveries or run errands for our organization! As a driver of our Chick-fil-A Fleet Vehicles, it is imperative that you drive responsibly to care for our guests, our community, and our team. Before being permitted to drive, you must fill out the following registration form. Upon receiving your registration, you will be notified by the Operator of your approval to drive the Chick-fil-A Fleet Vehicles. Driving the Chick-fil-A Fleet Vehicles without approval from the Operator will result in termination.

Please verify that you are 18 years of age or older.
Please verify that you have a valid driver's license.
Note: A valid driver's license is one that has not expired as of the date of signing this agreement.
I will use due care and caution in the operation of the vehicle and will strictly comply with all safe driving rules of the road, including all speed limits, posted directional signs and parking regulations.
Please state your compliance.
Under no circumstances will I operate the vehicle while under the influence of drugs or alcohol or when my physical or mental condition may be otherwise impaired.

I understand I am prohibited from carrying any non-Chick-fil-A team members or off duty Chick-fil-A Team Members as passengers.

I will notify the Operator immediately if I am involved in an accident of any nature.

I will under no circumstances use a cell phone or other electronic communications device while in driving operation of the vehicle.

I will not use any form of tobacco, cigarette or e-cigarette in the vehicle.

I will remove all personal items and trash from the vehicle at the end of each delivery.
Please state your compliance.
I will be mindful that my driving represents the Chick-fil-A brand and will be careful to be courteous to other drivers and pedestrians at all times.

I will notify the Operator immediately if there are any problems/part malfunctions on the vehicle.

Ensure gas is at least 1/2 tank at end of of shift.

I understand I will be financially responsible if I incur traffic/moving violations while in operation of the vehicle.

I understand that once my delivery is completed and I am awaiting another order that my responsibilities will shift towards front of house support. I will not wait in my car at the store.
CFA Vehicles are only to be used for work purposes.
While driving I will ensure I do not have any ear suppressors (airpods, headphones, other items that may impair my hearing) to ensure I can be aware of the road and surroundings
During Peak hours 11:30-1:30, I will park in the back parking lot to ensure I don't add to customer traffic congestion
I'll verify all my KitchenIQ checklists are completed when I am finished with my shift and that I've checked all needed messages from leaders/delivery captain
I understand by signing my name below I am signing in agreement to the statements made in this registration form. I also understand by typing my name below that failure to comply with any of these statements at any time of operating the van is just cause for discipline up to and including termination.
