Please note that Liberty Hill Pharmacy will only administer vaccinations on Tuesdays, Thursdays, and Fridays between 11 am and 6 pm. 

When you arrive for your appointment, please ensure you bring with you the following items:

• Drivers License or Identification card
• Medical Insurance 
• For 2nd dose or booster shot, your current vaccination card (Liberty Hill Pharmacy will not provide replacement cards)

 

The FDA has approved the following:

Children between the ages of 5 to 11 can receive COVID-19 Vaccinations. We are happy to provide this service to you and your family; however, we ask that someone accompanies the child(children) to comfort the spirited and or frightened ones. We will be unable to administer the vaccines if your child is uncooperative.

2nd booster shot has been approved for age 50 + and is immunocompromised for Pfizer, and Moderna only. 

We are no longer able to offer the Johnson and Johnson vaccine.  We do not know if we will be getting another shipment.

We encourage you to download the V-Safe tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccine.  Through V-Safe, you can quickly tell CDC if you have any side effects after getting the vaccine.  V-Safe will also remind you to get your second COVID-19 vaccine.
