Please read carefully, sign this Final Report, and return to Hunger Solutions Minnesota (HSM) no later than July 17, 2024.
By signing this application, the applicant agrees to use Minnesota Food Shelf Program (MFSP) funding in accordance with Federal, State & Local laws, rules & regulations and terms and conditions as required by HSM and the Minnesota Department of Human Services, Office of Economic Opportunity (MN DHS OEO)
The grantee:
1. Is a non-profit tax exempt 501(c)(3) organization, is affiliated with another organization with tax exempt 501(c)(3), or is a Tribal Nation.
2. Will adhere to regulations as detailed in MN state statute 256E.34 FOOD SHELF.
3. Will use MFSP funding on allowable expenses including, but not limited to: food, operating costs, personnel, space/rental, utilities, transportation, refrigeration and storage needs, mobile food costs, technology, PPE, and training.
4. Will submit a Final Report documenting how the grant money was spent (HSM and/or MN DHS OEO may request funds back if Final Reports are not received by due date)
5. Will retain all grant related documents for 7 years including, but not limited to source documentation for grant funds (receipts, invoices, etc.).
6. Will participate in site visits (on-site or virtual) by HSM and/or MN DHS OEO and will address any corrective actions proposed by HSM and/or MN DHS OEO
7. Will distribute food to persons in need at no charge regardless of race, color, religion, creed, national origin, sex, marital status, status with regards to public assistance, membership or activity in a local commission, disability, sexual orientation, age or other criteria unrelated to need.
8. Will not foster or advance religious or political views.
9. Has a stable address and provides direct service to individuals and families.
10. Will make services accessible to people with a disability or have a plan in place to serve disabled clients in compliance with the Americans with Disabilities Act (ADA).
11. Will make voter registration materials available to eligible participants (not applicable for Tribal nations).
12. Will make SNAP materials available to eligible clients participants (not applicable for Tribal nations).
13. Will be responsible for proper handling & safeguarding of client information and will get required permission before sharing any personal information including names and addresses.
14. Will comply with all requirements relating to food safety and food recalls with 7 CFR Part 250.4(c)(2).
15. Will submit accurate service statistics to their regional food bank by the 10th of each following month including the number of children, adults, and seniors served, households served, pounds distributed.
16. Must also distribute TEFAP.
17. Will maintain compliance with the MN TEFAP Policy and Operations Manual. Including, but not limited to TEFAP eligibility and intake.
18. TEFAP providers cannot require participants to provide any of the following in order to receive food:
ID for anyone in the household
Proof of income (TEFAP regulations do not allow providers to ask for or collect income documentation like pay stubs, or other income data from program participants,)
Proof of address (TEFAP providers are required to provide food assistance to all households that express need whether or not they reside within the provider’s “self-identified” service area.)
19. Will provide the same level of service to all program participants.
The grantee will repay funds if there is discrimination in service or if there is failure to comply with other terms and conditions of the contract.
Failure to abide by the terms of this agreement will make the grantee ineligible to receive state funds.
