    This tool is designed to help the Nominating Committee assess the level of experience, skill and competency each director has in various areas, as well as the overall composition of the Board as it relates to diversity. Please indicate your level of competence from 1-5 based off of the scale below.

    Scale

    1 -  No personal knowledge or experience in this skill

    2 - Some knowledge and experience but no direct involvement with this skill

    3 - Has knowledge and experience with this skill enough to understand issues and implications from a strategic perspective

    4 - Has significant knowledge and experience (up to 5 years) including direct experience as a senior leader in this skill

    5 - Has substantial experience and knowledge (5+ years) in this skill, including supplying strategic input to similar situations or organizations of similar scope and complexity as a senior leader in this skill

    1 - Limited Capability: Little to no experience related to the competency or skill

    2 - Basic level: Exposure to competency/skill is limited to education, self-learning
