I understand that information which I have provided may be verified by contacting persons and organizations with whom I had contact or who/which may have information concerning me.

I authorize the Global Methodist Church to conduct a background check consisting of, without limitation, the following information: county, state, and/or federal criminal records; Department of Motor Vehicle records, records of judgments and/or liens, educational history, employment history, or other information. I also authorize, without reservation, any person, agency, or other entity contacted by or on behalf of the Global Methodist Church to furnish the described or similar information.

I waive any and all causes of action that I may have against the Global Methodist Church, its members, employees, and agents, and any person, agency, or other entity providing information in connection with the background check for libel, slander, defamation, intentional or negligent infliction of emotional distress or for any other injury of any kind or nature caused by the gathering or supplying of the descried or similar information.

I authorize all such persons to treat a photocopy of this authorization as though it were an original, executed authorization.
After your application has been reviewed, you will receive an email from our background check provider, MinistrySafe. Please watch your inbox for this email, which will have the subject 'Global Methodist Church Background Questionnaire Notification' and will include a link to a secure form on MinistrySafe's web site. You must submit a background check form to be approved for membership in the Global Methodist Church.

Please note that although many applications are reviewed within the first week they are received, it is not unusual for some to take several weeks, so please be patient.
