Registration Form

One time registration Fee of $25 per student or $40 per family will be included with tuition upon registration to secure a spot in class. Class sizes are kept small so reserve a spot early to avoid disappointment. Please note that tuition and registration fee is neither refundable or transferable.

Upon completing registration, please deposit at least 50% of tuition to reserve a spot in class. Full Payment is due 1 week prior (Dec 30) to term commencement on Jan 6. There is a 5% sibling discount applied to second/ lesser tuition. Please note that Ballet Etobicoke reserves the right to cancel any class due to low enrollment at which time a refund will be administered.

I UNDERSTAND that participating in  training, ballet class, programs or workshops offered by Ballet Etobicoke during which I will receive information and instruction about health and fitness. I recognize that dance and fitness programs require physical exertion, which may be strenuous and may cause physical injury and I am fully aware of the risks and hazards involved.

I UNDERSTAND that it is my responsibility to consult with a physician prior to and regarding me and/or my child’s participation in pilates, training, ballet class, programs or workshops. I represent and warrant that I and/or child is physically fit and has no medical condition which would prevent full participation.

I UNDERSTAND that in consideration of being permitted to participate in training, ballet classes, programs or workshops, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I and/or my child might incur as a result of participating in the program. I knowingly, voluntarily and expressly waive any claim I may have against Ballet Etobicoke or Royal York Rd United Church for injury or damages that I and/or my child may sustain as a result of participating in the program. 

I UNDERSTAND and authorize that Ballet Etobicoke may take and use photographs/videos of me and/or my child for purposes of record keeping, advertising and marketing. I recognize that I do not have any rights to these photographs/videos and will not be compensated for the same. 

I UNDERSTAND that I, my heirs, or legal representation forever release, waive, discharge, and covenant not to sue Ballet Etobicoke & instructors or Royal York Rd United Church for any injury or death caused by negligence or other acts. I have read the above release and waiver liability and fully understand its contents. I voluntarily agree with the terms and conditions above and further certify that the above information is correct and accurate.
