I declare that the statements above are true and correct, and I agree that the landlord may terminate my agreement entered into in reliance on any misstatement made above. If for some reason we may not contact certain references above, please explain why.

PRE AUTHORIZED DEBIT AGREEMENTS ARE REQUIRED TO BE SIGNED BEFORE KEYS ARE GIVEN TO TENANTS.
