SentriLock Lockbox Transfer Request Form

Request is hereby made by the Original Owner to transfer ownership of the SentriLock lockboxes listed herein to the new owner named below.

IN RESULT OF THE DEATH OF LOCKBOX OWNER: Agent’s heirs or estate will retain ownership of all agent lockboxes. If the heirs/estate choose to transfer lockboxes NSBAR will need verification from the executor of will of power to do so.
