The information contained in this application is used only for the purpose of placing a puppy and will not be used elsewhere. Once we receive your completed application, we will contact you via phone to discuss your application and answer any questions you may have. If your application is approved, a non-refundable reservation fee of $500 will be due within 24 hours from our call and secures your spot on the reservation list.
