Transfer Credit Approval Request Form

This form is for full-time students who wish to transfer credits toward their degree requirements at CMKL University. Students seeking to transfer credits to CMKL courses or competencies must meet the eligibility requirements outlined below.

Undergraduate Students

Grade Eligibility:

• Students must have a cumulative GPA of at least B (or 3.0 on a 0–4 scale).
• Courses with non-eligible grades will not be considered for transfer.
• Students must complete the course before submitting the transfer credit request.

Transfer Credit Review Process:

• The evaluation process begins once all required documents are received.
• The transfer credit review typically takes 7–14 days to complete.
• Students are highly recommended to submit their transfer credit request within their first enrolled semester.

All submitted documents must be reviewed and compared against CMKL competencies/course to determine course equivalency and eligibility for transfer credit.

The result will be notified via email. The final decision is at the discretion of the program.

• Courses with non-eligible grades will not be considered.
• Courses must be relevant and aligned with the student’s graduate program at CMKL.
• All courses must be completed prior to submission of the transfer credit request.

• The evaluation process begins once all required documents are received.
• The transfer credit review typically takes 7–14 days to complete.
• Students are highly recommended to submit their transfer credit request within their first enrolled semester.

• A certified digital transcript in PDF format is acceptable to upload in the next section. Hard-copy transcripts and other academic records must be submitted in person or mailed to the Academic Affairs and Admissions Office after submitting the online form by 15 days.

• Course Syllabus: This document outlines the objectives, topics covered, and expectations for the course.
• Course Materials: Attached are various materials such as assessments, examples of exams, assignments, or other relevant resources that will aid in understanding of the course content.
• Instructor's Resume or CV: Enclosed is the resume or curriculum vitae of the instructor, providing information about their qualifications, academic background, and relevant experience.
