Parent Consent Form – 2023 SEDA College Swimming Carnival

I(Parent/Carer) give permission for my son/daughter to attend and participate in the SEDA College Swimming Carnival on the Friday 16th Feburary 2023 at HBF Stadium (100 Stephenson Ave, Mount Claremont WA 6010). I am aware the college has qualified Bronze Star and S.T.A.R.T (School Teacher Aquatic Rescue Training) staff in attendance who will be supervising and monitoring the aquatic environment at the carnival. If you child’s medical details have changed prior to the date of this information and consent form, please contact the college to update your child’s records.

I have discussed with my child the need for mature behaviour throughout the day.I authorize the College staff attending the carnival to decide for the welfare of my child. This includes any medical treatment that may need to be administered to my child.I agree that my child may be using public transport to get to and from the carnival as per normal college practiceI understand that if my child exhibits any unnecessary behaviour, then arrangements will be made for my child to be removed from the carnival and sent home.I acknowledge in the case of an emergency, SEDA College WA staff will conduct appropriate first aid if required and act in the best interests of safety of my child at all times.
