The Sew Modest Summer Camp is an 8 week program from June 9th - July 28th. In order to successfully complete the program, students should attend all sessions. Missing more than 2 sessions and/or excessive tardiness (being late 3 times or more) will result in the student being expelled from the program. No refunds will be provided.* This field is required.

What's included: Students will be provided with tote bags, workbooks, fabric (main and lining), zippers, reusable sewing patterns, use of all hand tools and machinery. They will be responsible for bringing all their supplies to class each week. What's NOT included: Jewelry, Headwear (hijab, crowns, etc.) or petticoat. * This field is required.

Excessive Late Pick up: If you are running late for pick up please text me at (313)870-8802. We have a 10 minute grace period, after which you will incur late pick-up fees. This fee is charged at a rate of $20 for every 10 minutes per child. * This field is required.Students will not be allowed to participate in the graduation program until this balance is paid.

SignaturePHOTO/VIDEO RELEASE FORM I hereby give permission for images of my child(ren), captured during the Sew Modest Summer Camp and Fashion Show through video, photo and digital camera, to be used for the purposes of promotional material and publications for Sew Modest LLC and Sew Modest Summer Camp, and waive any rights of compensation or ownership thereto.In my capacity as the Minor's parent/guardian, I agree as follows: 1. I hereby consent to the recording of statements, photographs, and/or audio or video recordings taken of the Minor or me by Sew Modest LLC employees and contractors. 2. All statements, photographs, and/or audio or video recordings taken of the Minor or me, by Sew Modest LLC employees and its contractors, may be used by Sew Modest LLC for promotional, commercial or other purposes, anywhere in the world in its sole discretion. Neither the Minor nor I shall have any right to control the use or publication by Sew Modest LLC of the statements, photographs, and/or audio or video recordings. 3. All statements, photographs, and/or audio or video recordings taken of the Minor or me by Sew Modest LLC employees or its contractors, shall be the sole property of Sew Modest LLC. Neither the Minor nor I shall receive any compensation in connection with use of these statements, photographs, and/or audio or video recordings for promotional, commercial or other purposes. 4. On behalf of the Minor and myself, I hereby release, waive and discharge any claims of any kind or nature arising out of or relating to the use of the statements, photographs, and/or audio or video recordings against Sew Modest LLC or any person or firm authorized by Sew Modest LLC to publish said materials (“Publisher”), Such release, waiver and discharge shall also extend to all affiliated companies, shareholders, directors, officers, employees, agents and assigns of Sew Modest LLC and any Publisher. 5. GENERAL WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT

There Are Absolutely No Refunds, No Transfers, No Exchanges, No Make Up Classes And No Rescheduling.

Class will start and end promptly according to the schedule, Students may arrive no more than 15 minutes before the scheduled start time. Being late for class and/or missing sessions will result in you missing critical instruction that will not be reviewed.

I Am Aware That My Participation In This Activity May Be Hazardous And I Am Voluntarily Participating In This Activity With Knowledge Of The Danger Involved And Hereby Agree To Accept Any Risks Of Damage To Myself, My Personal Property Or Personal Injury.

I, the undersigned participant, for myself and for my heirs, executors, legal representative, successors and assigns, hereby waives all claims and/or course of action, including negligence, against LaTerry Aaron, Sew Modest LLC, Sew Modest Studio, the Trustees, all officers, directors, employees and agents, arising out of or in any way connected with my participation in all courses, workshops and activities held at Sew Modest LLC.

By Signing below, I Certify That I Have Carefully Read This Agreement And Fully Understand Its Contents. I Am Aware That This Is A Waiver of Release Of Liability And Assumption Of Risk Agreement Between LaTerry Aaron, Marwan Mohsin, Sew Modest LLC, Sew Modest Studio, the Trustees, all officers, directors, employees and agents and Myself.

CORONAVIRUS/COVID-19 WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that Sew Modest LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that Sew Modest LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other clients and their families.

I voluntarily seek services provided by Sew Modest LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment. Including wearing a mask at all times.

I attest that:

* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* I have not traveled internationally within the last 14 days.

* I have not traveled to a highly impacted area within the United States of America in the last 14 days.

* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.

* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold Sew Modest LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Sew Modest LLC. I understand that this release discharges Sew Modest LLC from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Sew Modest LLC. This liability waiver and release extends to the studio together with all owners, partners, and employees.
