Junior Volunteer Application Agreement and Waiver

Junior Volunteers are between the ages of 15 and 17 years of age and do not have contact with animals unless permission is granted via Waiver and Release Form, which would allow them to hold non-aggressive cats. You must be able to volunteer on the same day, on the same shift on a weekly basis. The Junior Volunteer program cannot be used for court-ordered, community and/or school service hours. The Parma Animal Shelter is completely operated by volunteers. Our mission is to help lost, homeless and abandoned animals found in the City of Parma. Our goal is to reunite lost animals with their humans and to find good homes for those we shelter.

• Wash dishes, litter pans and scoops.
• Sweep main cat room floor as needed.
• Assist in main cat room with giving cats their canned treats, filling water bowls and food dishes, cleaning out litter boxes, recording daily log and disinfecting cat/kitten cages as needed.
• Cannot open any cat cages without direction of adult cat volunteer.
• Bring recyclable aluminum cans inside to wooden fenced area.
• Take trash to dumpster.
• Disinfect exercise cages as directed.
• Fill food and litter bins in main cat room. Put laundry away in correct area.
• Always be polite and courteous when talking or dealing with the public. Direct public to proper shift leader.

A parent or legal guardian MUST sign this Waiver before you can volunteer. 

represent that I am the parent or legal guardian of {name}, a minor child, with full authority to supervise and enter into agreements on the minor child's behalf.

I fully understand the risks associated with handling and being in the vicinity of animals, and particularly animals which may be ill, injured, neglected or abused.  Thus, in consideration of being offered the opportunity for th eminor child to participate in Parma Animal Shelter ("PAS) as a junior volunteer ("JV), I agree on behalf of the minor child as follows:

1. JV will follow the rules. JV will abide by all policies, procedures and instructions as adopted by PAS or described to them by its agents, with the understanding that these rules may change from time to time.

2. I authorize emergency treatment. I give permission to PAS in its discretion, to call the police, fire/rescue, or emergency medical services and to transport the minor child to any hospital or medical center in the event of a medical emergency. I authorize emergency medical transport and treatment rendered to the minor child.

3. JV will make no public statements or contracts. JV has no authority to make public statements on PAS's behalf, to enter into contracts for PAS, or to otherwise obligate PAS in any way.

4. JV is not an animal cruelty or other violent offender. I certify that JV has never been charged with or convicted of any offense related to the cruelty, abuse, neglect or abandonment of animals, or any other violent or sexual criminal offense of any kind.

5. JV can be terminated. I understand that PAS may terminate the JV's activities at any time, with or without cause. Upon termination, or upon my voluntary resignation, JV will immediately return all animals, equipment, records and other property owned by JV.

6. JV's image may be used. I support PAS's mission, therefore I irrevocably grant permission to PAS to use for art, promotion, advertising, trade, or any similar lawful purpose whatsoever, any photograph, video, sound recordings, or quote taken of or made by the minor child relating to their volunteer activities with the organization without prior review, reimbursement, or compensation of any kind.

7. I accept the risks, On behalf of myself, the minor child, and our heirs, assigns, executors and administrators, I hold harmless and indemnify PAS, its directors, officers, agents, volunteers and employees, from any and all losses, claims, actions or proceedings of every kind and character, including claims for negligence and for damage of any kind, including damage to property, personal injury or death to me or to third parties which may arise directly or indirectly from my presence at properties controlled or used by PAS, their handling or being in the vicinity of animals, or in any volunteer activities. Since JV is not an employee of PAS, I understand that there is no worker's compensation or insurance coverage for any injury, illness, loss or damage arising out of the JV's activities. PAS encourages all volunteers to maintain their own medical, property and life insurance coverage while serving as a volunteer, as all costs for injury or loss are my personal responsibility. For parent/legal guardian: I hereby certify that my minor child is fully covered under my personal/group medical insurance policy with 

8. Remedies. PAS may bring an action at law or in equity in a court of competent jurisdiction to enforce the terms of this Agreement, to enjoin the violation, ex parte as necessary, by temporary or permanent injunction, to recover any damages to which it may be entitled for violation of the terms of this Agreement. PAS's rights under this paragraph apply equally in the event of either actual or threatened violations of the terms of this Agreement. I agree that PAS's remedies at law for any violation of the terms of this Agreement are inadequate and that PAS shall be entitled to the injunctive relief described in this paragraph, both prohibitive and mandatory, in addition to other relief to which PAS may be entitled, including specific performance, without the necessity of proving either actual damages or the inadequacy of otherwise available legal remedies. PAS's remedies described in this paragraph shall be cumulative and shall be in addition to all remedies now or hereafter existing at law or in equity.

9. Binding Agreement. This Agreement constitutes the entire understanding between the parties. This Agreement shall be construed according to Ohio law, and shall be enforced by the proper court with jurisdiction over Parma, Cuyahoga County, Ohio. This Agreement may not be amended or modified without a writing signed by all parties.

The undersigned represents he or she, acting as parent or legal guardian, consents to the minor child taking part in PAS Junior Volunteer Program and on behalf of the minor child to the terms set forth above.
