Summer Camp Program: Request for Registration

All sessions are 2.5 Hours Per Day for 1 Week (5 days: Monday - Friday) equating to a 12.5 hour week for a rate of $450 / student / session with all equipment (bow, arrows, quiver, safety, etc) provided

We HIGHLY suggest against having any student sign up for / attend more than 1 session since the program outline and learning syllabus remains the exact same.  (see "Daily Camp Schedule 2023 Outline" section here: https://www.southbayarcherylessons.com/summer-camp-program/)

The form below is to request your space for registration only and does not guarantee your child a spot in the program.  Your child's registration spot will not confirmed until you complete all of the the following

1. Fill in the below form details which includes a waiver and COVID outline
2. Receive the Invoice from Square for the spot (after this form is received and information contained is processed by Coach Lani)
3. Pay the full payment before the Cancellation / Refund Cutoff Date (see Cancellation / Refund Policy here) 
4. Receive official range confirmation after full payment is received 

Due to high tournaments in the summer of 2023, there are very limited Camp Session weeks for South Bay Archery Lessons as the two Head Range Coaches required for hosting the camp will be traveling itermittently throughout the summer for a few days at a time with our competative students.  We apologize for this inconvenience in advance and hope to have more program session opportunties available later in the year 2023 or during Summer 2024.  To off-set this, we have opened more spaces per session than previous years with more instructors.

The below option are as back up options only.  Your additional option choices selected below, will not be invoiced unless approved by the parent/guardian and range coaches/instuctors first.

PHOTO RELEASE
Photographs and videos are routinely taken at events. I release the use of any images taken at this event for the purposes of photographing, video recording or streaming the event and promoting archery, but not for commercial purposes. With my signature below, I agree that images that are taken at this event by or on behalf of the event organizer may be used without compensation or additional permission.

CODE OF CONDUCT AND CODE OF ETHICS
I agree to be bound by the USA Archery Code of Conduct and Code of Ethics, and understand that my participation in this event is contingent upon my adherence to the Code of Conduct and Code of Ethics. The Athlete Code of Conduct and Code of Ethics may be viewed at usarchery.org.

WAIVER AND RELEASE OF LIABILITY AND ASSUMPTION OF RISK ("Release")

1. I understand dangers may exist or be caused by actions or inactions of the participants, and the actions or inactions of others, while participating in the archery event to which this Release applies (the "Activity"). I understand the nature of archery activities and acknowledge my experience and capabilities and believe I am qualified to engage in and conduct the Activity. I further acknowledge that I am aware that the Activity may be conducted in facilities open to the public during the Activity. I further agree and warrant that if, at any time, I believe conditions to be unsafe, I will immediately discontinue that part of the Activity, which involves the unsafe condition.
   
   
2. I FULLY UNDERSTAND that archery activities involve risks and dangers of BODILY INJURY, INCLUDING PERMANENT OR PARTIAL DISABILITY, PARALYSIS, OR DEATH OR OTHER HARM ("Risks").
   
   
3. I hereby agree that as organizer of this event I have/will use due diligence to ensure the archery range is secured and safe to avoid potential injury to participants and spectators and to avoid potential harm to the facility.  I have taken the necessary steps to provide proof of insurance coverage for the venue if so required, and agree that all participants will sign the USA Archery waiver form prior to participation.
   
   
4. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO HOLD HARMLESS South Bay Archery Lessons LLC, Paul Farbman, the California Indoor Archers, Gagos V Limited Partnership, and USA Archery and its administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, and advertisers from all liability, claims, demands, losses, or damages on account caused or alleged to be caused in whole or in part by (a) my acts or omissions in organizing or conducting the Activity and (b) the negligence of the Releasees or otherwise, including negligent rescue operations or emergency medical treatment, and further agree that if, despite this release, anyone makes a claim against any of the Releasees named above, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE OR COST ANY MAY INCUR AS THE RESULT OF ANY SUCH CLAIM WITH RESPECT TO WHICH THIS RELEASE APPLIES.

COVID-19 SAFETY AND HEALTH REQUIREMENTS ("Release")

1. I FULLY UNDERSTAND that I and any participating (Archer / Student) and non-participating (Specator) members of my scheduled group will be required to adhere to all of the most updated "Range Policy: COVID-19 Safety Requirements & Updates" as advised on the range website (or alternatively here, if the primary website is undergoing updates) 
   
   
2. I FULLY UNDERSTAND that should I and any participating (Archer / Student) and non-participating (Specator) members of my scheduled group fail to adhere to any of the most updated "Range Policy: COVID-19 Safety Requirements & Updates" as advised on the main range website (or alternatively here, if the primary website is undergoing updates) while within the building will result in myself or those in breach of the range policy being asked to immediately leave as a breach of health or safety requirements as outlined within this document and NO refund will be issued. 
   
   
3. I FULLY UNDERSTAND that I and any members of my group that show signs of sickness (sneezing, coughing, fever, etc) that may be deemed as related to COVID-19 symptoms, will not be allowed to enter the building and will be immediately asked to leave for safety and health reasons.
   
   
4. I FULLY UNDERSTAND that any additional, previously unscheduled participating (Archer) individuals that wishes to join after the initial scheduling was confirmed MUST be formally requested to be added to your session prior to the start of your session to ensure invoicing and scheduling is appropriately updated prior to final payment, if it is within the space allowances of the range. I acknowdge that I will request it by emailing the range through the website contact form or emailed directly contact@southbayarcherylessons.com only to ensure my request is received.  Text and voicemail / phone call will not be accepted as they cannot be tracked for reference in our range CRM.
   
   
5. I FULLY UNDERSTAND that any previously scheduled participating (Archer) individuals that are unable to join after the initial scheduling was confirmed MUST be formally requested to be removed from your session prior to the start of your session to ensure invoicing and scheduling is appropriately updated prior to final payment. I acknowdge that I will request it by emailing the range through the website contact form or emailed directly contact@southbayarcherylessons.com only to ensure my request is received.
   
   
6. I FULLY UNDERSTAND that I and any members of my scheduled group will be required to wait outside of the building at a safe distance from the entrance to the range until the time of our scheduled session to avoid close contact with or blocking the exitway for any previously scheduled session participants who will be exiting the building prior to our incoming session.  Additionally, we acknowledge that we will not be permitted to enter the building until one of the range coaches or workers has approved our entry.
   
   
7. I FULLY UNDERSTAND that any balances required to be paid at the end of a lesson (minus any required deposit to hold a private lesson, event or camp scheduled or registered for) must be paid on a device that belongs to either me or one of the other scheduled members of my group, instead of using the range payment device. Balance payment can be done from any smart device (tablet or phone) from the invoice link that all scheduled sessions received at the original time of confirmation.  Should I need to be resent this link, I acknowdge that I will request it by emailing the range through the website contact form or emailed directly contact@southbayarcherylessons.com only to ensure my request is received.
   
   
8.
