  • Hospitality House Youth Development is dependent on outside funding. Answering the following questions is necessary to receive the funding that will provide program income for HHYD to better serve our families.

    Please read the below expectations of parents and students and select yes or no if you agree to abide by those expectations.
  • Parent/Guardians Expectations

    Guardians are expected to attend all activities as scheduled (i.e. orientation, parent conferences, family fun events, and other planned activities). 

    Attendance: 

    Guardians are to encourage their children to attend the after school program daily for the duration of the hours per day to recieve the full benefits of the program. In the event your child is going to absent for a period of time, please let us know as soon as possible. Call 612-522-4485 and leave a message with the front desk person. You can also send us a written notification, please include the student's name, your name, and phone number where you can be reached. 

     Daily Pick-up: 

    All students must be picked up by 6pm. Guardians, if you cannot pick-up your child at the end of the appropriate session/atheltic event, please call or send a note if someone other than you (or the persons listed on the application) will be pickin up your child(ren).

    A late fee will be applied beginning five minutes passed the scheduled pick up time; for example 6:05-6:10 is $5.00. Parents MUST call if you are going to be late for acceptable reasons.

    To assure that a safe, productive, learning environment and that everyone is treated with respect, dignity and kindness. 

    All students are expected to...

    • Participate in all program activities
    • Follow directions
    • Make an effort to complete all tasks
    • Take responsiblity for one's own actions
    • Be respectful to self, others, and property
    • Talk postively, appropriately with kindness and care to others
    • Dress in a manner appropriate for a learning community
    • Understand that all behavior has consequences 

    Unacceptable and inappropriate behaviors include but are not limited to: 

    • No profanity, no swearing, and no put downs
    • No possession or use of tobacco, alcohol or other drugs on HHYD property
    • No firearms or weapons on HHYD property
    • No inappropriate touch to others (fighting, kissing, grooming others, pulling of others clothing/hair)
    • No threatening or engaging in any other forms of violence or other behavior that will be disruptive to the safety, activities participation or learning of self and others
    • No use of electronic devices unless during allowed times. These include, but not limited to games and cell phones.

  • Release and Indemnification: 

    In consideration for my child to be permitted to participate in Hospitality House programs, I/we, on behalf of myself/ourselves, my child, and my respective agent, assign beneficiaries, heirs, personal representatives, successors, trustees, and other representatives herbey assume the risk and agree to release, hold harmless, discharge, indemnify, and defend the Hospitality House, its directs, officers, employees, agents, and volunteers from and against any and all liability, including all causes of action, claims, damages, participation in Hospitality House activities, including, without limitation, my child's attending, being at, being involved with, and/or participating in Hospitality House activities, including, without limitation, any claims arising in connection with any of the affirmations, authorizations, commitment, permissions, releases or undertakings I/we have made in or through this Consent and Release. These indemnification, hold harmless, and defence terms shall apply and be fully enforceable even if such injury, illness, or disease, or damage arises in whole or in part from the negligence of Hospitality House, its directors, officiers, employees, agents, and volunteers and shall include, without limitation, the reasonable attorneys' fees of Hospitality House, its directors, officers, employees, agents, and volunteers associated therewith.

  • Coronavirus/Covid-19 Assumption of Risk and Waiver

    This is for students enrolled in our in person programming.
  • I, * (Full Name of Parent or Guardian), am the parent or legal guardian of * (child(ren)).

  • IN CONSIDERATION for being permitted to utilize the services, and programs of Hospitality House Youth Development ("Hospitality House") and/or for my children listed above to so participate for any purpose, including, but not limited to, programs and activities affiliated with the Hospitality House. The undersigned, on behalf of himself or herself and such participating children and any personal representatives, heirs, and next of kin (hereinafter referred to as "the undersigned"" hereby acknowledges, agrees and represents that he or she has inspected and carefully considered such premises, equipment and facilities and/or the affiliated program and that the undersigned finds and accepts same as being safe and reasonably suited for the use or participation by the undersigned and such participating children. 

    The undersigned acknowledges that the novel coronavirus (“Covid-19”), has been declared a worldwide pandemic by the World Health Organization and many infections have been confirmed throughout the United States, including several cases in Minnesota. Covid-19 is extremely contagious and is believed to spread mainly from person-to-person contact. In accordance with the most recent guidance and protocols issued by the World Health Organization (WHO), the Centers for Disease Control and Prevention (CDC), the Minnesota Department of HEalth (MDH), for slowing the transmission of Covid-19, the undersigned hereby agrees, represents, and warrants that neither the undersigned nor such participating children shall visit or utilize the facilities, services, and programs of Hospitality House (other than any exclusively online services and programs) within 14 days (i) returning from highly impacted areas subject to a CDC Level 3 Travel Health Notice, (ii) exposure to any person returning from areas subject to a CDC Level 3 TravelHealth Notice, or (iii) exposure to any person who has a suspected or confirmed case of Covid-19. The CDC Travel Health Network is continuously updating this list and the undersigned agrees that they are aware of this list and the countries listed. 

    The undersigned hereby agrees, represents, and warrants that neither the undersigned nor such participating children shall visit or utilize the facilities, services, and programs of Hospitality House if he or she (i) experiences symptoms of Covid-19, including, without limitation, fever, cough or shortness of breath, or (ii) has a suspected or diagnosed/confirmed case of Covid-19. 

    The undersigned agrees to notify Hospitality House immediately if he or she believes that any of the foregoing access/use restrictions may apply. 

    Hospitality House has taken certain steps to implement recommended guidance and protocols issued by the Public Health Agencies for slowing the transmission of Covid-10, including, without limitation, the access/use restrictions set forth above. The undersigned acknowledges and agrees that Hospitality House may revise its procedures at any time based on updated recommended guidance and protocols issued by the Centers for Disease Control and the Minnesota Department of Health, and further agrees to comply with Hospitality House’s revised procedures prior to utilizing the facilities, services, and programs of Hospitality House. 


    The undersigned further acknowledges and agrees that, due to the nature of the facilities, services, and programs offered by Hospitality House, although social distancing of 6 feet per person among children and staff in various activities and programming is encouraged and recommended, it may not be possible. 


    The undersigned fully understands and appreciates both the known and potential dangers of utilizing the facilities, services, activities and programs of Hospitality House and acknowledges that use thereof by the undersigned and/or such participating children may, despite Hospitality House’s reasonable efforts to mitigate such dangers, result in exposure to Covid-19 ,which could result in quarantine requirements, serious illness, disability, and/or death. 


    IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER HOSPITALITY HOUSE FOR ANY PURPOSE INCLUDING, BUT NOT LIMITED TO, OBSERVATION OR USE OF FACILITIES OR EQUIPMENT, OR PARTICIPATION IN ANY ON SITE OR OFF SITE PROGRAM OR ACTIVITY AFFILIATED WITH HOSPITALITY HOUSE, THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING: 


    THE UNDERSIGNED, ON HIS OR HER BEHALF AND ON BEHALF OF SUCH PARTICIPATING CHILDREN, HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE. Hospitality house, its directors, officers, employees, volunteers and agents from all liability to the undersigned or such participating children and all personal representatives, assigns, heirs, and next of kin of the undersigned or such participating children for any loss or damage, and any claim or demands on account of any property damage or any injury to, or an illness or the death of , the undersigned or such participating children (or any person who may contract Covid-19, directly or indirectly, from the undersigned or such participating children) whether caused by the negligence, active or passive, of Hospitality House or otherwise while the undersigned or such participating children are in, upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with Hospitality House. 


    THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS Hospitality House, its directors, officers, employees, volunteers and agents, and each of them, from any loss, liability, damages or costs they may incur, whether caused by the negligence, active or passive, or otherwise while the undersigned or any participating child is in, upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with Hospitality House. 


    The undersigned understands and agrees that Hospitality House is not required to provide insurance to cover the undersigned or such participating children in the event they suffer illness, injury, death, property loss, theft or damage of any sort upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with Hospitality House. 


    The undersigned agrees and acknowledges that use of Hospitality House facilities and services and participation in Hospitality House programs, may involve inherent danger and risk, including, without limitation, the risk of physical illness or injury, death or property damage. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR, AND RISK OF ILLNESS, BODILY INJURY, DEATH OR PROPERTY DAMAGE to the undersigned or such participating children due to negligence, active or passive, or otherwise while in, about or upon the premises of Hospitality House and/ or while using the premises or any facilities or equipment thereon or participating in any program or activities affiliated with Hospitality House the undersigned acknowledges that any illness or injuries that the undersigned or such participating children contract may be compounded by negligent first aid or emergency response of the Releases and waive any claim in respect thereof. 


    THE UNDERSIGNED further expressly agrees that the foregoing ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY, AND INDEMNITY AGREEMENT  is intended to be as broad and inclusive as is permitted by the laws of the State of Minnesota and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue to full legal force and effect. 


    I HAVE CAREFULLY READ AND VOLUNTARILY SIGN THIS ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY, AND INDEMNITY AGREEMENT AND FURTHER AGREE THAT NO ORAL REPRESENTATIONS, STATEMENTS OR INDUCEMENT APART FROM THE FOREGOING WRITTEN AGREEMENT HAVE BEEN MADE. I AM AWARE THAT BY AGREEING TO THIS AGREEMENT I AM GIVING UP VALUABLE LEGAL RIGHTS, INCLUDING THE RIGHT TO RECOVER DAMAGES FROM HOSPITALITY HOUSE IN CASE OF ILLNESS, INJURY, DEATH OR PROPERTY LOSS OR DAMAGE, INCLUDING, FOR THE AVOIDANCE OF DOUBT AND WITHOUT LIMITATION, EXPOSURE TO COVID-19 AT HOSPITALITY HOUSE OR ACTIVITY OR PROGRAM AND ANY ILLNESS, INJURY OR DEATH RESULTING THEREFROM. I UNDERSTAND THAT THIS DOCUMENT IS A PROMISE NOT TO SUE ANDA RELEASE OF AND INDEMNIFICATION FOR ALL CLAIMS. IF SIGNING ON BEHALF OF MINOR: I ALSO UNDERSTAND THAT THIS AGREEMENT IS MADE ON BEHALF OF MY MINOR CHILD(REN) AND/OR LEGAL WARDS AND I REPRESENT AND WARRANT TO HOSPITALITY HOUSE THAT I HAVE FULL AUTHORITY TO SIGN THIS AGREEMENT ON BEHALF OF SUCH MINOR(S).

  • Minor Electronic Communications Consent and Release Form

    This form provides your permission, along with your consent and release, for your child to participate in electronically based programs and activities.

  • I authorize and consent to staff or other leaders of Hospitality House Youth Development (“Hospitality House”) to electronically communicate with my Child. I understand that such communications are for Hospitality House purposes only and may involve group communications relating to Hospitality House
    programs and activities. I understand that communications may be accessible or viewable by others. Further, I understand that such electronic communications may be made via text, email, telephone and cell phone, social media, video conferencing, and other electronic means (referred to as “Electronic Communications”). Examples of Electronic Communications include, but are not limited to, Zoom Video Communications, Facebook, Skype, FaceTime, and Video Calling on Facebook. I understand that these Electronic Communications are web-based platforms operated by third-parties, its own privacy terms and conditions to which members must adhere and which parish or diocese cannot control or assume
    responsibility.

    I understand that if any staff or other leaders knowingly communicate privately with my child as a part of his or her duties for or on behalf of Hospitality House, reasonable steps will be taken to inform me of the communication content, though not necessarily via the same technology.

    I acknowledge that to review or receive public communications shared via social media with my Child, I may need to become a fan or follower of the same social media.

    I understand that communications or posts may be accessible or viewable by others who are also fans or followers of the same social media. I understand that without this consent, my child will not be able to participate in Hospitality House programs and activities available through Electronic Communications.

    If I choose to rescind my authorization and consent provided in this Electronic Consent and Release Form, I agree that I will inform the Hospitality House in writing and that my rescission will not take effect until it is acknowledged by Hospitality House.

    I understand, however, that it may not be possible to recall any work, photos or videos that have been published as part of the program(s) prior to receipt of my written rescission.

    I have read this Consent and Release Form and have had the opportunity to consider its terms and understand them. I verify that I have read and voluntarily agree to the terms and conditions of the Hospitality House Youth Development Minor Electronic Communications Consent and Release Form.

    I further hereby hold harmless, release and forever discharge the Hospitality House Youth Development and its employees, volunteers, agents, and legal representatives from, and shall indemnify them against, all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators or
    any other person(s) acting on my behalf or on behalf of my estate have or may have because of my Child’s participation in the program(s) and through my authorization, consent and release herein.

    I have read this Hospitality House Youth Development Minor Electronic Communications Consent and Release Form, I fully understand it, and I voluntarily agree to be bound by its terms. I represent and certify that I am the parent or legal guardian of the minor.
