DAA Nomination

Distinguished Alumni Award Nomination
Since 1977, more than 100 distinguished alumni of the Anne Spencer Daves College of Education, Health, and Human Sciences have been recognized for excellence in their careers, support of their communities, and service to Florida State University. The Distinguished Alumni Award is selected by a panel of alumni, retired faculty, and College staff to represent diverse accomplishments and celebrate the breadth of experiences possible with a degree from Florida State University.

Nominations are open through November 1, 2025. Any application material sent after November 1 will not be considered.

Required Materials
- Nominator Information: Name, address, phone number, email
- Nominee Information: Name, address, phone number, email
- Statement of Nomination: 150 words or less that explains why the nominator feels the person is a qualified candidate
- References: Three references, their contact information, and at least one letter of recommendation from one of the references
- Supporting Documentation: Current curriculum vita or resume of nominee and any documentation further supporting nominee’s accomplishments and qualifications for the award (e.g. other letters of recommendation, announcement of other recognitions, news articles, etc.)
