Gel-X Extension Liability Waiver

I acknowledge by signing this release that I have been given the full opportunity to ask any and all questions which I might have about obtaining Gel-X from CYNDO.ESTHETICS hereafter called Cynthia and that all of my questions have been answered to my full and total satisfaction.

The following statements are CYNDO.ESTHETICS policies and I agree to the following rules and regulations:

• I understand that no guests are allowed in the studio. Guests will be asked to leave. 

• I understand that I must come in with clean, bare, HEALTHY nails unless booking for a soak-off. I will NOT work on damaged nails.

• I understand I have a 48-hour policy. I have 48 hours to let my Gel-X Nail technician know if there is anything wrong with my set for a complimentary fix. No questions asked. After 48 hours, I will be charged a nail fix fee of $5 for each nail/new set.

• I understand that pricing is subject to change and that there are no refunds. No exceptions.

• By sending me your deposit, you have agreed and read all policies. No refunds, and no exceptions.

Gel-X: $30 deposit
All deposits are NON-REFUNDABLE. A deposit must be sent immediately after booking. Appointment will be canceled if no deposit is sent. Deposits will go towards your remaining balance.

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the Gel-X artist's day that could have been filled by another client. As such, we require a 24-hour notice for any cancellations or changes to your appointment. You may reschedule your appointment ONCE with the same deposit. Clients that provide less than a 24-hour notice or miss their appointment will have their deposit forfeited. Please send a courtesy message if you're running late. If you're more than 15 minutes late, your appointment will be automatically canceled and your deposit will be forfeited. No-shows will not be tolerated and will be banned from booking future appointments.

I understand and agree to allow CYNDO.ESTHETICS to apply Gel-X nails on my nails. I understand that CYNDO.ESTHETICS is not certified/or licensed in doing nails/Gel-X nails. I understand there are risks associated with having Gel-X applied to and/or removed from my nails, and that notwithstanding the utmost care in the application or removal of these products, there still exist risks associated with the procedure and product itself. I understand that as part of the procedure, damage and/or infection may occur. I understand and I agree that if I experience any of these issues with my nails, I will contact my technician and have the nails removed immediately and consult with a physician at my own expense. This agreement will remain in effect for this procedure and all future Gel-X procedures conducted by my technician. I also agree to defend, indemnify and hold harmless the Gel-X artist from any and all claims, actions, expenses, damages, and liabilities, including reasonable attorneys’ fees which might be asserted against her as a result of my having this procedure performed, or my purchase of these Gel-X products from her.

I hereby grant the Gel-X artist the full right to take, publish and reproduce photographs of my hands and/or fingers, both before and after this procedure, for any advertising, education, or other purposes whatsoever, including the right to retouch these photographs as deemed necessary by the Gel-X artist. I further expressly assign any copyright in these photographs to the Gel-X artist. I also grant my consent for the Gel-X artist to use my image and likeness as contained in these photographs for any advertising or other purposes

I agree to follow the care and maintenance instructions provided by the Gel-X artist for the use and care of my Gel-X extensions, and that if any follow-up care is required due to my own mistake or negligence, or failure to follow these instructions, this will be at my own expense and risk. There are no guarantees for the bonding time length of the Gel-X nails.

I have read and completed the Gel-X Intake Form in its entirety and in truth. I acknowledge that I have been advised of the potential harmful or negative side effects that the Gel-X procedure or removal may cause to those who have specific medical or skin conditions. I further state that I have no known medical condition that might be aggravated by the procedure or any medical condition that would prevent me from complying with or heeding the Gel-X artist's instructions or these warnings.
