Confirmation 2024 Application Form

Dear Parents / Carers,

Thank you for your interest in enrolling your child to our Confirmation Preparation Course 2023.

Before starting to fill this form, please kindly note that:

Baptism Certificate: you will be asked to upload a copy of your child's baptism certificate to this site. This field is set mandatory therefore you will not be able to submit the application without the relevant certificate in hand.
First Holy Communion Certificate: we expect child to enrol to our course has already received his/her First Holy Communion in the past. You will be asked to confirm the date of your child's First Holy Communion Date and aslo to upload a copy of your child's FHC certificate to this site. If your child has not received his/her FHC at the time of application, please advise us by selecting the most appropriate button. Our Catechist team will ensure that your child will receive FHC when he/she is confirmed.
Missing Certificate: If your child was baptised and / or received first holy communion at Christchurch, and you are unable to locate the original certificate(s), the Parish Office will assist you to reissue a certificate at the admin cost of £10 per certificate. Please email the Parish Office (elthamconfirmation@rcaos.org.uk) with your child's full name, DoB, the date of the sacrament (Baptism/First Holy Communion) included.
Course Fee: the cost to enrol your child to the course is £35 per child. This fund will cover bookets for the course, renting of rooms and all other sundry costs incurred for the Parish to run the course.
Attendance to the course: this year's course is relatively short, therefore your child is expected to attend every session without fail. The full dates & time is available to be downloaded from the Parish website (https://www.christchurcheltham.org.uk/Confirmation/).
1. Introduction Evening & Group Session 1 (Wednesday, 13th March 2024 at 6.30pm at the Church) - the first part of the evening (approximately 30 minutes) must b attended by both your child and his/her parent/carer. Failing to attending this evening will prompt the catechist team to remove your child form the course immediately.
2. Enrolment Mass (Sunday, 21st April 2024 at 6.30pm) - attendance by both your child and his/her parent/carer to this Mass is mandatory. If you already know that your child or his/her parent/carer will not be available to attend this Mass, please do not register your child to this course.
Selection of Mass Date, Sponsor and Confirmation Name: the selection fo Mass Date, of Sponsor and of Confirmation Name will be offered once your child has attended the enrolment Mass at a half-way through the course programme.
