Iconnect Credentials and Receipt Form

The Agency for Persons with Disabilities requires that all Employees/Contractors working with medwaiver recipients receiving APD services use the Iconnect system. Of those services, Respite and Personal Supports must use the CyberArk Identity app in conjunction with Iconnect when working with Medwaiver consumers. The app works by tracking through your devices GPS, your location at the start of your shift and at the end of your shift.

The usage of this app is required. Upon acceptance of a position with Connext Care, LLC, you are agreeing to:

1. Maintain a working cellphone or tablet with service.
2. Have your location tracked by GPS while with a Connext Care, LLC consumer.
3. Clock In and out as applicable to your schedule.
4. Complete your Progress notes and any other required documentation.
5. Acceptance that your Paycheck reflects the hours that were successfully and correctly entered in the Iconnect system and/or Cyberark app.
