TATTOO REQUEST FORM

Please use the following form to submit a tattoo design and idea. I highly recommend reading through this information prior to sending a request in, as I won't reply to any requests that don't fall in line with my policies. (Also be sure to check your junkmail for a response).

Blush charges a starting price of $80, and increases from there depending on the size/detail of the tattoo. All tattoos will have a 50% deposit attached to their booking, and that will be put towards the total cost of your piece.

Cancelations OR rescheduled appointments with less then 48 hours notice, will lose their deposit and require a new one to book in. Being over 10 minutes late for your appointment may require the need to reschedule at the artists discretion. If so, you will lose your deposit, and will require a new one to rebook.

No guests are allowed at your appointment unless you are getting a tattoo together. I work in a small space and I focus better with less in the room :)

A tattoo submission does not mean your design will be chosen. You will be contacted to proceed with a booking. Please keep an eye on your junk mail.

Blush DOES NOT tattoo the following:

Anyone under the age of 18

Pregnant or nursing
Undergoing Chemotherapy
Prone to keloid scarring

If you don't have a ready idea, or I cannot comprehend what you are trying to convey, your request will not be replied to, please be as specific as possible.

Your appointment is accepted based on design approval. Changing your design could result in a cancelled appointment if the design does not suit my style/portfolio. If you'd like to change your design you must contact me a minimum of 2 weeks before your appointment. Changes will be discussed based on time availability and style. Any additional changes made on a peice already sent in and approved will have a charge of $20. Day of changes are not allowed.

Custom pieces will have an additional $20 fee.
