This reservation form is for adult groups of 15 or more, including colleges and universities. If you are a K-12 school, summer program, or other youth-serving organization, please complete this form.
Groups of 15 or more who book in advance enjoy a discounted rate of $8 per adult. Bus drivers are free.
NEW! Docent-led tours for adult groups will become available on June 1. Docent-led tours will be charged $25-50 in addition to admission, payable upon entry or invoiced after the visit. Higher education classes and tribal nations are waived the fee for a docent-led tour, but still must pay admission.
FAM requires a completed request submitted no fewer than three weeks prior to your visit date. If you are having trouble completing this form, please email tours@famok.org.

NEW! Optional, docent-led tours for adult groups of 15 or more provide personalized gallery experiences for groups. FAM assigns one docent per 15-20 individuals so small groups benefit from a more relaxed and conversational experience. The nonrefundable fee for a docent-led tour is due upon booking; admission is paid at the desk upon entry or invoiced after your visit.

Expectations and Guidelines for your visit:

Fees for docent-led tours are non-refundable except in the case of museum closure; transfers are not permitted without advance request to tours@famok.org at least two business days prior to the visit.
Groups more than 30 minutes late may have their docent-led tour canceled without refund.
Groups must make prior arrangements to visit the restaurant.
Please be mindful and respectful of other tour groups or individual visitors.
Outside food and drinks are not permitted, with exceptions for medically necessary items.
Please leave your pets at home—only service animals required due to a disability are permitted.
No smoking or vaping is permitted on FAM grounds.
No firearms, weapons or drones are allowed.
Visitors are not allowed to climb the side of the Mound; the Mound will close during inclement weather.
All youth 17 and under must be accompanied by an adult.
Soliciting is not permitted.
