I hereby affirm that all information provided above is accurate. By signing below, I acknowledge:
* I am responsible for my own tent(s), table(s), chair(s), and other items required for my comfort and the successful display of my product or service.
* I understand all vendors are to arrive at least one hour prior to the gates opening for each Levitt AMP Selma Concert Series event.
* If I will sell a product or service, I am responsible for submitting the $100 vending permit fee to the City of Selma through the Office of Tax & License prior to each Levitt AMP Concert Series event. I will do so online at www.Selma-AL.gov or by dialing (334) 874-2112.
* I understand I am to present the vending permit issued by the City of Selma to the appropriate Levitt AMP Series personnel BEFORE I set up my vending area.
* If I am a nonprofit organization, I am not required to purchase a vending permit from the City of Selma. I will instead present my agency credentials to the appropriate Levitt AMP Series personnel BEFORE I set up my display.
* I release all liability for injury and all other mishaps from the Levitt Foundation, Levitt AMP Selma, TRHT Selma, the Black Belt Community Foundation, the Selma Center for Nonviolence, the City of Selma, and the Selma-Dallas County Chamber of Commerce and Tourism Information.
