Must submit the following: -- A 3x5 color photo of applicant must be submitted via email or uploaded with this online application) - 2 Letters of References from a Teacher at his/her existing High School/College - A copy of proof of acceptance to your desired college or vocational school - A signed Letter of Consent from the Student, Parent or Guardian of the recipient to publish recipient’s name, photo and the name of the attending school (in newspaper and on website)
