1. By completing this form and paying the entry fees, you will receive a PLAY Pass, granting admission to PLAY at Okada Manila, our indoor playground facility.
2. A PLAY Pass is inclusive of a 2-hour playtime inside the facility. An extension of playtime shall be charged the equivalent of a PLAY Pass. Children aged 5 and under and children with special needs must be accompanied by an adult at all times and must remain inside throughout the duration of the visit. Children found without an adult may be removed without refund or compensation.
a. An adult shall attend to and supervise a child aged 3 and under at all times (1:1).
b. An adult can attend to a maximum of three (3) children aged 4 and 5 (1:3).
3. Children aged 6-17 are allowed to enter without being accompanied by an adult, but an adult must be present during the child’s check-in and check-out.
4. Children who are sick or have had a fever (and not on fever-reducing medication) within the last 24 hours, or have had any recent exposure to a contagious illness shall not be allowed entry.
5. Parent/guardian who registered the child at entry is the only one allowed to leave and exit with the child.
6. No drop-offs and no in and out privileges.
7. Parents/guardians are encouraged to claim their children 15 minutes before closing time. If the child is not claimed 15 minutes after closing time, the management shall be constrained to turn over the child to the relevant government agency or appropriate non-government agency.
8. All guests, whether children or adults, shall wear socks while in the PLAY area. Socks are available for purchase from the retail counter. Guests using the slides are advised to wear long sleeved shirts.
9. Do not leave valuables in the day lockers or shoe cubbies.
10. Please ensure that the child is not wearing loose jewelry and clothes worn must not have any metallic rivets, buttons on the back pocket, or design that may scratch other children or surfaces.
LOCKER RENTAL
1. The use of lockers at PLAY is strictly for registered guests only.
2. To rent a locker, a valid government-issued ID must be provided to the staff assigned at the exit in exchange of a locker key. One (1) ID is to one (1) locker key.
3. Locker key must be returned to the exit staff after use, and surrendered ID shall be returned in exchange thereof.
4. The owner of the surrendered ID shall be the only one allowed to claim upon return of the locker key.
5. Lost locker keys are charged at Php1,000 and shall be paid by the guest at the PLAY reception.
6. The use of the lockers is solely the responsibility of the user. Okada Manila is not liable for any loss or damage to property of the user.
