Emergency Information

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Onward Speech and Language Therapy during the selected class. In exchange for the acceptance of said child’s candidacy by  Onward Speech and Language Therapy, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Onward Speech and Language Therapy, and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected classes.

In case of injury to said child, I hereby waive all claims against Onward Speech and Language Therapy, including all therapists and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. 

I understand that the Onward Speech and Language Therapy holds no liability for any accidents that may occur on the property of the class. I am fully responsible for any injuries or accidents. I will fully supervise my child at all times.

Food Allergy Waiver

This is to inform you of a specific risk associated with food allergies. Onward Speech and Language Therapy will make reasonable accomodations to offer allergy-friendly foods and products. Cross-contamination may occur when preparing foods and in the play environment. We will take extra precautions to not cross contaminate but cannot guarantee that traces of allergens are not present in the foods/products that are used and served. This may include but is not limited to nuts, gluten, dairy, eggs, etc. If allergies and exposure of allergens are of concern for yourself/child, then please be aware and make an informed decision. By completing the consent below, you are acknowledging that you have read and understood this notice and exclude Onward Speech and Language Therapy from any liability.

Cancellation

If needing to cancel or reschedule, please communicate this at least 24 hours before the class time.  A credit will be given for the next class. No refunds will be provided.

Media Release

I authorize Onward Speech and Language Therapy to take photographs and/or videos of myself/my child to be used for promotional material including, but not limited to website and social media.

I further understand that if the photographs and or videos are used, my name and/or other identifying information will be kept completely private and confidential. I do not expect compensation, financial or otherwise, for the use of these photographs/videos.

Confirmation

By acknowledging and signing below, I am delivering an electronic signature that will have the same effect as an original paper signature.
