    Person Centered Approach Policy

    Outcome to achieve: Personalized Care or treatment specific to the needs, preferences and wishes of the person involved

    Their participation shall be paramount and the staff who shall be performing the care and support assessment shall ensure provision for the consumer to participate in the meeting using any communication aid or tool they may need for communication.

    Consumers are given all information needed to help them to make an informed decision about the Company and what we can offer and the opportunity to gain support from alternative methods or resources should it be needed, and we are unable to provide. Any reason for not being able to meet their needs will be explained to them at assessment or when a decision is made so that they are kept informed.

    For Consumers who lack capacity to make specific decisions we will look to gain support or consent from a lawful representative unless one has not been appointed in which we will look to get support to make decisions in the best interest of the Consumer.

    Under APD the support plan is an encompassing term for the following documents which must be considered as a whole to determine the full plan.

    • Consumer personal details and support network
    • Daily tasks and outcomes to achieve
    • Moving & positioning assessments
    • What is important to me - personal assessment of wishes and preferences
    • Medical conditions and the effects they place on the Consumers
    • Environmental risk assessment
    • Financial support and safety assessment
    • Medication support and management
    • Nutritional support
    • Mental capacity and advocation support in place.

     

    CC aims to ensure that the following shall apply when creating a personalized plan on behalf of the consumer

    • Is indeed duly created and agreed with the full agreement and support of the Consumer
    • Has been developed through study of the above-mentioned documents and in consideration of their findings
    • Details any specialist needs or communication requirements
    • Identifies areas for promoted independence and flexibility where needed/wished for
    • Has been developed solely based on the Consumer's expressed wishes but with considerationgiven to relatives, representatives, friends and professionals if appropriate and agreed. It is a Policy of this CC that a Consumer always has the opportunity of some time alone with a CC manager so that he/she may express their views privately should they wish.

    • Is developed in consideration of the expressed wishes of the Consumer with regard to risk taking but within the constraints of the law and not contradicting the other policies and procedures of the company
    • Considers the complexity of service requirements and each plan reflects this complexity in its content
    • Is reviewed at least annually or as a change of circumstance dictates. Consumers are to be made aware that they may request a review at any time. When reviewing, the Company shall seek permission of the Consumer to invite all those to be present at initial review
    • Is kept updated, records are kept of any revisions made and ensure that any agreed actions are carried through. Changes are made available to a Consumer in a format and language understandable to them
    • Is signed by means of agreement by the Consumer or their representative. A copy of the plan is given to the Consumer and a second copy is retained in the office for reference constrained by the company's Records Policy.

    The company acknowledges that all services must -

    • Be person-centered
    • Involve relevant others agreed by the Consumer
    • Be multidisciplinary if necessary, in consideration of social and healthcare needs
    • Provide opportunities for relevant persons to manage the service user's care
    • Be evidence based
    • Set objectives and consider ways to achieve those objectives
    • Be realistic
    • Be reviewed
    • Be actioned
