Your Rights
Your have the right as a consumer to the following Rights
- You have the right to choose the Support and Services
- You have the right to be given the services needed to obtain you own home or apartment
- You have the right to confidentiality
- You have the right to privacy consistent with the need for safety, without denying you the dignity of risk.
- You have the right to be treated fairly by me with respect and dignity
- You have the right to be free from neglect, abuse, exploitation, or any form of corporal punishment
- You have the right to file a grievance and may appeal our decision
- You have the right to deny or consent to services offered
- You have the right to be informed about who must or will pay for services
- You have the right to look at your records
- You have the right to know why you might be denied services
Your Responsibilities
As a consumer of Support Services, you have the following responsibilities:
- You must actively participate in the services provided
- You must attend scheduled service activities and meetings
- You must allow us to collect funding for the services we provide
- You must maintain the confidentiality of other people in the program at all times
- You cannot use or possess weapons, drugs, or alcohol in the presence of your provider
- You must not act violent or abusive toward other people.
*If you fail to meet your responsibilities you may be discharged from the program
