Please complete this application form for each person (player & goalie) who will attend the Soccer Skills Factory - Summer Soccer Camp.

July 31th-August 5th, 2023
8 AM- 1 PM younger group

4 PM -9 PM older group 

The younger group (U7-U12). Early drop-off is at 7:30 AM. Pick up 01:00 - 01:30 PM. All kids MUST be picked up no later than 01:30 PM.

The older group (U13-U19). Early drop-off is at 3:30 PM. Pick up 09:00 - 09:30 PM. All kids MUST be picked up no later than 09:30 PM.

This is an allergy-friendly camp.

Each package includes: 

• The five-day or three-day training program (3 sessions per day + creative workshops & active leisure time);
• Fruits & snacks;
• Official Soccer Skills Factory photo;
• Written feedback from the coach;
• UEFA Certified Coaches directly from Europe;
• The methodology used by the most advanced European grassroots and elite youth programs;
• Official Soccer Skills Factory Uniform (jersey, shorts, and pair of socks);
• Official Soccer Skills Factory Certificate;
• Strict hygiene and health protocols;
• Experienced monitors for every 15 participants;
• Group Level: The players will be evaluated for grouping according to their level and age;
• Age groups: U6s, U7s & U8s / U9s, U10s, & U11s /  U12s, U13s/ U14s, U15s & U16s / U17s & U18s. The age groups stated are guidelines, not absolute rules;
• Goalkeeper Training: 3 sessions, plus they’ll participate in drills to develop footwork;
• Trip opportunities: Players who participate in our Training Camps will have the opportunity to be selected to participate in the European Soccer Experience Tour, which is an exclusive and unique experience to visit some of the most iconic soccer sites in Europe.

Informed Consent and Acknowledgement

I hereby give my approval for my personal/child’s participation in any and all activities prepared by the b1keeper Academy during the selected sessions. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless the b1keeper Academy and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected training sessions.

In case of injury to said child, I hereby waive all claims against the b1keeper Academy, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

BY AGREEING ABOVE AND SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT IS EQUIVALENT TO AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

CANCELATTION POLICY 

- After the payment confirmation, we will assign your child to the camp programme.
- This is a soccer camp, and you have to commit for the whole camp in order to see the soccer improvement of your child (Space is limited -First Come First Serve)
- The coaching fee is included in all sessions for camps.
- Cancellations received within 7 days of registration and before the 1st session may be eligible to receive a full refund less 30% service fee.
- Cancellations received after 1st practice or 1st session are NOT eligible to receive a refund.
- If your child is sick or does not attend the class, session or camp for any reason, NO compensation will be made from the paid fees for the time missed. Note the payment is for a period of the whole camp commitment and can not be prorated for any reason.
- In case of a refund, the amount will be paid after the soccer camp program by mailing a check or through Paypal less Paypal fees.

- No refunds or reschedules if some of days/sessions are canceled due to inclement weather.
