Effective Date* Rogue Active Duty Animal Rescue, Corp. (hereinafter known as ‘RADAR’), in consideration of the sum of $ ($400 - $600)* (non-refundable), the receipt of which is acknowledged by a RADAR representative, releases full responsibility and ownership of Pet's Name*
to the undersigned adopter.

As the adopter, I agree to abide by the terms of the adoption set forth via this document. My signature and initials below indicates that I am at least 21 years of age and intend to be the legal owner and the person responsible for the care and well-being of the animal I am adopting from RADAR. I understand that any misrepresentation of the facts in my previously provided adoption application and correspondence with the rescue will invalidate the adoption agreement and give RADAR the right to reclaim the dog without refund of the adoption fee.

INTIAL HERE

This animal is healthy to the best of RADAR's knowledge. I understand that the animal has been given certain basic inoculations and other medical treatment as indicated necessary by a veterinarian, and all medical records and conditions will be disclosed to me. Although the animal appears well, an appointment for a consultation with my veterinarian should be made as soon as possible.

I realize that this animal may have undiagnosed medical problems or may be incubating common parasites. I accept responsibility for the continued veterinary care of this animal. I understand that RADAR will not be able to provide further assistance with diagnosis and management of this animal, but will take the animal back if I am unable to care for it. I understand that arrangements for return may take up to 3 weeks so that a new foster home can be found. The animal I am adopting is accepted by me "as is.' I agree to provide yearly checkups, shots, flea/tick and Heartworm prevention for the remainder of this animal's lifetime. I understand that RADAR makes no representations or warranties regarding the current or future medical condition of the animal and is not responsible for additional medical care after the adoption has been finalized.

While most adult animals have been sterilized, I understand that if I have adopted a dog/cat or puppy/kitten that has not been altered I agree to have this dog/cat spayed or neutered by the age of 6 (six) months or within 60 (sixty) days of adoption if an adult. An adoption of an unaltered pet assumes an additional fee of $100 which will be refunded at the time that proof of spay/neuter is presented. I agree to send a copy of the veterinarian issued spay/neuter certificate or invoice to: RADARescuer@gmail.com. All age-appropriate dogs have been tested for Heartworms. If found positive, treatment has been started and necessary follow up treatment has been explained to me. If found negative, we strongly urge you to retest in six months since there is an incubation period, and we are not responsible for any treatment after the adoption date. We require that all dogs be kept on regular Heartworm, flea and tick prevention.

I understand that RADAR makes no representations or warranties regarding this animal's behavior with adults, children, or other animals, nor provides guarantees as to the animal's characteristics, personality or training. I understand and agree that RADAR is not responsible for any injuries or property damage resulting from the possession or ownership of this animal. See attached temperament evaluation if applicable.

As this animal's new owner, I accept full responsibility for maintaining a reasonable level of care including food, water, grooming, adequate sanitation, exercise, attention, safety, shelter and prompt veterinarian diagnosis and treatment of medical conditions. This animal will live as a family companion and reside inside my home. This animal will not be kept as an outside dog/cat neither on a tether nor in a fenced area. The animal will not be allowed around children without adult supervision, nor be allowed to ride loose in the back of a truck, nor remain unattended in any vehicle in extreme temperatures and/or for more than 30 (thirty) minutes. If applicable, I will license the animal in the town/city/State where the animal will reside, according to local laws and statutes. I will ensure that the animal wears an identification tag in case they should become separated from me for any reason. I agree to promptly contact RADAR if the animal is lost for more than 24 hours. If I am found to be in violation of any of the above, I acknowledge and accept that a RADAR representative will immediately take possession of the animal and criminal charges may be filed.

Should for any reason the dog/cat be returned either by the owner or as required by RADAR for breach of contract, all associated costs will be the responsibility of the adopter. I.e. provide proof of maintained and current vetting (including but not limited to: spay/neuter, vaccinations, Heartworm prevention). For dogs: if no proof of continued Heartworm prevention exists, a current Heartworm test must be provided. If the animal tests positive, the adopter will be responsible for the cost of Heartworm treatment. Please note arrangements for return may take up to 3 weeks so that a new foster home can be found. The animal may not be sold or given to a new owner without RADAR's prior written approval. RADAR reserves the right to an ongoing review of this adoption and to follow up on any complaints or reports to protect the welfare of every animal we adopt out. If the terms of this agreement are not satisfactorily upheld by the adopter, and/or if any misrepresentations have been made to RADAR in order to obtain this animal, RADAR reserves the right to demand the immediate return of the animal. I agree to relinquish custody immediately upon request without the need for further legal writ, court order or law enforcement intervention should the terms of this agreement be violated.

I, on behalf of myself and all persons holding residence in the household where the animal will reside, hereby accept possession ofpet's name . I hereby release, indemnify and discharge Rogue Active Duty Animal Rescue, Corp. and its directors, officers, employees and agents from and against any liability, injury or damage to any person or property caused in the future by this animal, and from and against any causes of actions, suits or demands whatsoever that may arise as a result of such injury or damage.
