Please complete this registration form then complete the following contract for quick processing so that we can get your furkid into our facility to play! If you aren't ready to register your dog please fill out our contact form so we can answer any questions you may have! Please note that dogs are required to come in for an evaluation before they are permitted into our Daycare program. Vaccinations are also required before dogs are accepted. Vaccinations must meet your veterinarian's protocols based on your dog or puppy's age. This includes Rabies; Distemper/Parvo, Bordetella, Leptospirosis, Canine Influenza, Flea and Tick (12 months). Puppies are required to be spayed or neutered by six (6) months to attend Daycare or Boarding as well as a negative stool. Unaltered dogs CAN visit the Grooming Salon. I fully accept all reasonable risks of illness or injury that may be incurred during normal daycare activity. Such risks include, but are not limited to: cuts, scrapes, and other injuries resulting from play and possible contagious illnesses. I understand that I am fully responsible for all medical costs performed by the veterinarian.
