I understand that if I am approved for volunteering, the Volunteer Agreement represents the legal contract between a volunteer and One Life at a Time Animal Rescue, Inc.

VOLUNTEER AGREEMENT

In consideration of this opportunity to volunteer, I agree to the following terms
and conditions, intending to be legally bound by them:

1. I will abide by the mission, rules, regulations, policies and programs of One Life at a Time Animal Rescue Inc., while I am a volunteer. I also agree to observe and follow all signs and instructions posted.
2. If I stop being a volunteer for One Life at a Time Animal Rescue, Inc. for any
reason, or upon the request of One Life at a Time Animal Rescue, Inc. or its
representatives at any time, I will promptly return all of One Life at a Time Animal Rescue Inc’s supplies, equipment, records, money and other items in good clean condition.
3. I understand that the activity of working with rescue animals is hazardous and involves contact with animals that may be unpredictable. As such, One Life at a Time Animal Rescue, Inc. cannot be held liable for injuries or accidents that may occur as a result of working with the animals. I assume the risks of being bitten, scratched, kicked, injured or frightened by animals, and any infections or complications that may occur as a result, in connection with my volunteer work for One Life at a Time Animal Rescue, Inc. One Life at a Time Animal Rescue Inc. is not liable to me for any injuries, damages, liabilities, losses, judgments, costs or expenses whatsoever, which I might suffer or sustain in connection with my volunteer activities for One Life at a Time Animal Rescue Inc. I agree that I will not hold One Life at a Time Animal Rescue, Inc., including but not limited to its Directors, Officers, and their spouses of each, financially or legally responsible for any injuries, damages or claims, and agree to waive all rights of Subrogation and legal defense from One Life at a Time Animal Rescue, Inc. I release One Life at a Time Animal Rescue, Inc., its officers, directors and their spouses of each from any and all liability of damage, injury, illness, property damage.
4. I agree that if I see any dangerous situation I will notify One Life at a Time
Animal Rescue Inc. immediately.
5. I understand and agree that One Life at a Time Animal Rescue, Inc. may refuse volunteer applications for any reason.
6. If I will be sheltering or providing foster care or boarding any of One Life at a Time Animal Rescue Inc’s animals, I agree to fill out and sign the separate Foster Application and Agreement.
7. I have accurately and truthfully completed this Volunteer Application and
Agreement.
8. Any modification to this Agreement must be in writing and signed by both
parties.

I have read this Application in its entirety, and I agree that all statements contained in this document are made by me, and are truthful and I agree to the terms of the volunteer agreement. I make this statement under penalty of perjury under the laws of the state of Pennsylvania.
