Accreditation Support

This form is for the community to request support from Hockey Australia and partnered State & Territory Associations in regards to updating your HockeyEd profile. By completing this form, you will be requesting for an existing HockeyEd Accreditation to be added to your new HockeyEd Profile.
Accreditation Support Applications are processed based on the date received and can take up to four weeks to process.

Please allow four weeks for the accreditation to appear in your HockeyEd profile.

This will not affect your accreditation completion date.

In order to have your existing Accreditation added to your profile, users will need to ensure that they have created an account on the new HockeyEd platform.

To get set up on the new platform, follow the instructions at the bottom of this page: https://www.hockey.org.au/hockey-ed/

Once you have done this, be sure to take note of your new HockeyEd username. You can find this on the HockeyEd home page in the top left hand corner of the screen.

Your username will be in the following format: ha.firstname.lastname

Please note that we will not accept any submissions unless you have your new HockeyEd user name in this exact format.

In order to verify your existing accreditation, we will need a minmum of 1 (one) item of proof of your accreditaton.

Example documents that help support the existance of your old accreditations are:
- Completion Cerficate (including completion date)
- Practical Competency Checklists (including completion date)
- Receipt of Payment (this will be from paypal)
