Our training fees are $35 for a session.

Fees are payable monthly, (at the start of every month).

- New players fees:

If a player joins us midway through the term, fees will be prorated accordingly.

*Please note that we do not accept payment per session.

- Registration fee. (Only for new players).

A mandatory registration fee of $120 is required upon registration. This also cover's the player's training kit, (a top - shorts - socks).

PARENT'S DECLARATION.

Should LEADS UNITED FC be unable to contact me (the parent of a child), I give my permission to LEADS UNITED FC staff to seek medical assistance for my child if/when this is needed. I will bear full financial responsibility for any costs incurred.

LEADS UNITED FC reserves the right to use any photographs and/or videos taken at any of our training venues and league matches if any, for publicity purposes.

The person filling this registration form MUST be the parent of the child or the legal guardian.

MAKE-UP POLICY

We will notify parents at least 2 hours earlier if a session have to be cancelled.
Pare.nts should text the club's number (9376 3659) at least 2 hours earlier to inform that their children will miss a training session. Failing to report that, the full fee for that session will be dedacted from any payment made.

If the 2 hours notice is given early, any payment made for that particular session will be carried forward to the following month/term if possible.

If there are any missed sessions due to bad weather or low number of participants, that session will be carried forward to the following month or term.

A valid reason as a medical certificate is required in order to validate the absence to training of any player.

BLACKOUT DATES (no training days).

We recommend that parents inform the club at the begining of each term/month when making payment about any players/participants absence period during that term or month.

REFUNDS.

We only offer a refund if a family is relocating out of Singapore. (4 weeks notice prior to the date of departure is required).

OUR TERMS & CONDITIONS.

Football is a contact sport and as such, injuries do happen. As LEADS UNITED FC is extending it's reach to the community, using various locations as Playgrounds and Parks around the city, we promise to take every precaution to ensure that injuries do not occur during our training sessions and/or league matches.

By signing up to our services, I understand and agree that LEADS UNITED FC, it's staff and players will not be held responsible or liable for any permanent injuries, loss of property or loss of life of your child that may occur arising from any situation on and/or off the playing field, whatsoever circumtances during and after their time at any of our training sessions and venues.

LEADS UNITED FC and it's staff will not be held responsible or liable for any permanent injuries, loss of property or loss of life of any player's parent and/or relatives that may occur from any cause whatsoever during and after their time at any of our training venues and/or matches.

We strongly advise parents to get themselves covered with their own personal or family insurances in relation to any sports related injuries, loss of property and any other type of accident.
