  • Dependent Credits Questionnaire

    Please complete this questionnaire if ANY of the following apply:

    • you have children or dependents that are college age or younger
    • you have children or dependents who lived with you for more than half the year
    • you provide financial support to another relative who is not your child
    • you paid (or used loans to pay) for college or other higher education expenses for yourself, your spouse, or your dependents

    IRS regulations require paid tax preparers to perform a series of due diligence requirements for the Earned Income Credit, Child Tax Credit, and Head of Household Filing Status. 

  • *You can't claim the EIC for a child who didn't live with you for more than half of the year, even if you paid most of the child's living expenses*

  • *Sometimes a child meets the test to be a qualifying child of more than one person. However, only one of these persons can actually treat the child as a qualifying child. Only that person can use the child as a qualifying child.*

  • *Please include Form 1099-INT. You may not have received this form if interest was under $10. If you did not receive this form please select Interest was less than $10*

  • *Please include Form 1099-B or 1099-Combined*

  • *Please include Form 1099-G, this can be found and downloaded from the unemployment portal*

  • *Please include Form 1099-R*

  • *You may have received  Form 1099-R or Form 1099-DIV*

  • *Please include Form W2-G, Form 1099-MISC or 1099-NEC*

  • *Please include Form W2-G and a log of gambling losses which includes the date of loss, location of loss, & other relevant info. Without a log, losses cannot be deducted.

  • *Please include Form 1099-C*

  • *Please include a year-end summary from your crypto wallet(s). We may also require a report from a Crypto Tax Software, such as Koinly or Zenledger.*
